INSUFFICIENT RELIABLE EVIDENCE to RATE

• Allergic rhinitis (hay fever). It is unclear if oral astragalus is beneficial for allergic rhinitis. Details: Preliminary clinical research shows that taking a specific astragalus root extract standardized to contain 40% polysaccharides (Lectranal, Milsing d.o.o.) 160 mg orally twice daily for 3-6 weeks improves symptoms such as rhinorrhea, sneezing, and itching when compared with placebo in adults with seasonal allergic rhinitis (17355).
• Amenorrhea. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: One preliminary clinical study shows that taking a liquid decoction containing astragalus 30 grams, dong quai 30 grams, zizyphus 10 fruits, horny goat weed 15 grams, dodder 30 grams, and three slices of ginger, daily in two divided doses for 3 months, can increase menstrual frequency when compared to baseline in patients with amenorrhea (33050). The validity of this finding is limited by the lack of a comparator group. Additionally, it is unclear if this effect is due to astragalus, other ingredients, or the combination.
• Angina. It is unclear if oral astragalus is beneficial for angina. Details: A preliminary clinical trial shows that taking astragalus 20 grams orally three times daily for 2 weeks can improve cardiac output, but not diastolic function, when compared to baseline in patients with angina (33063). The validity of these findings is limited by the lack of a comparator group.
• Anthracycline cardiotoxicity. It is unclear if intravenous astragalus is beneficial for preventing cardiotoxicity due to anthracyclines. Oral astragalus has not been evaluated for this use. Details: A large unblinded clinical study in patients with breast cancer or lymphoma receiving a chemotherapy regimen containing epirubicin shows that intravenous use of astragalus 500 mg daily for 14 days during 2 courses of chemotherapy modestly attenuates the decline in left ventricular ejection fraction (LVEF) and reduces the occurrence of cardiotoxicity when compared with standard care (110479). However, this study was conducted in China, which limits the generalizability of its results.
• Aplastic anemia. It is unclear if intravenous astragalus is beneficial for aplastic anemia. There is insufficient reliable information about the clinical effects of oral astragalus for this condition. Details: Preliminary clinical research in patients 15 years and older with chronic aplastic anemia shows that giving intravenous astragalus in combination with stanozolol 2 mg three times daily produces greater improvements in symptoms and blood cell counts than stanozolol alone. Astragalus was given as 80-120 grams daily for repeated 15-day courses at 7-10 day intervals for at least 4 months (32840).
• Asthma. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Preliminary clinical research in children with mild asthma shows that taking a combination of astragalus, cordyceps, Radix stemonae (Bai Bu), bulbus fritillariae cirrhosae, and Baikal skullcap orally for 6 months does not improve asthma symptoms, measures of lung function, or total steroid use when compared with placebo (32935).
• Beta-thalassemia. Although there has been interest in using oral astragalus for beta-thalassemia, there is insufficient reliable information about the clinical effects of astragalus for this condition.
• Breast cancer. Although there has been interest in using oral astragalus for breast cancer, there is insufficient reliable information about the clinical effects of astragalus for this condition.
• Cervical cancer. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of 19 clinical trials in adults with cervical cancer who are receiving chemotherapy shows that the use of Chinese herbal medicines containing astragalus increases tumor response (complete and partial responses) and Karnofsky performance score and reduces chemotherapy-associated adverse effects when compared with chemotherapy alone (107479). However, most studies included in the meta-analysis were low-quality, and all studies were conducted in China. Higher quality studies in other geographic locations are needed to confirm these findings. Additionally, it is unclear if any effects are due to astragalus, other ingredients, or the combination.
• Chemotherapy-induced anemia. Oral or intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of low-quality clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy reduces the risk of anemia by 51% when compared with chemotherapy alone (102099). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Chemotherapy-induced diarrhea. It is unclear if oral or intravenous astragalus is beneficial for chemotherapy-induced diarrhea. Details: One preliminary clinical study shows that intravenous infusion of astragalus 40 grams in 250 mL of solution, given daily for 21 days during each of four courses of chemotherapy, reduces diarrhea by 59% when compared to chemotherapy alone (32747). Additionally, a meta-analysis of low-quality clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy reduces the risk of diarrhea by 45% when compared with chemotherapy alone (102099). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Chemotherapy-induced leukopenia. Oral or intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of low-quality clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy reduces the risk of thrombocytopenia by 41% when compared with chemotherapy alone (102099). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Chemotherapy-induced nausea and vomiting (CINV). Early research suggests that intravenous astragalus seems to reduce nausea and vomiting associated with chemotherapy use. It is unclear if oral astragalus is beneficial for CINV. Details: A meta-analysis of low-quality clinical research in Chinese patients with advanced non-small cell lung cancer shows that intravenous infusion of astragalus along with platinum-based chemotherapy reduces the risk of vomiting by 38% when compared with platinum-based chemotherapy alone (100698). Another preliminary clinical study shows that intravenous infusion of astragalus 40 grams in 250 mL of solution, given daily for 21 days during each of four courses of chemotherapy, reduces nausea by 36% and vomiting by 50% when compared with chemotherapy alone (32747). Higher quality studies are needed to confirm these results. The use of astragalus in combination with other ingredients for prevention of CINV has also been investigated. A meta-analysis of low-quality clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy reduces the risk of nausea and vomiting by 44% when compared with chemotherapy alone (102099). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Chemotherapy-induced nephrotoxicity. Oral or intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of low-quality clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy does not reduce the risk of nephrotoxicity when compared with chemotherapy alone (102099). The effects of astragalus when used alone are unclear.
• Chemotherapy-related fatigue. It is unclear if intravenous astragalus is beneficial for chemotherapy-related fatigue. There is insufficient reliable information about the clinical effects of oral astragalus for this condition. Details: One clinical study in patients with advanced cancer shows that treatment with an intravenous infusion of a partially purified astragalus extract (PG2, Pharmagenesis), 500 mg three times weekly for 4 weeks during chemotherapy, improves fatigue scores after one week, but not after two and four weeks, when compared with placebo (33034).
• Chronic fatigue syndrome (CFS). Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: One preliminary clinical study in adults with CFS shows that taking a 200 mL decoction containing astragalus 30 grams, kudzu 30 grams, codonopsis 15 grams, red sage 10 grams, aizoon stonecrop 15 grams, horny goat weed 10 grams, turmeric 10 grams, and calamus 10 grams, in two divided doses daily for 3 months improves fatigue symptoms when compared to baseline (32920). Another preliminary clinical study shows that taking 1.5 grams of a specific product (Myelophil, Samik Pharmaceutical Company) containing 66% of extracts of astragalus and danshen in a 1:1 ratio twice daily for 4 weeks improves fatigue severity when compared with placebo. However, taking 3 grams of the same product twice daily does not seem to have a benefit (32883). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Chronic kidney disease (CKD). It is unclear if intravenous astragalus is beneficial for CKD. There is insufficient reliable information about the clinical effects of oral astragalus for this condition. Details: A meta-analysis of preliminary clinical research in patients with CKD shows that using astragalus, usually as an injection, along with conventional treatment modestly improves creatinine clearance by 6 mL/min, serum creatinine by 0.25 mg/dL, and hemoglobin levels by 1 gram/dL when compared with conventional treatment alone (90660). However, the studies included in the meta-analysis were of low quality. Also, it is not known if astragalus reduces mortality or attenuates the progression to kidney failure.
• Cirrhosis. Intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. There is insufficient reliable information about the clinical effects of oral astragalus for this condition. Details: A meta-analysis of results from low-quality clinical studies shows that intravenous injection of a combination of astragalus and danshen for up to 90 days might decrease fibrosis when compared with control in patients with liver cirrhosis (90662). However, it is unclear if this is due to astragalus, danshen, or the combination.
• Colorectal cancer. Oral or intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of clinical trials in patients with colorectal cancer shows that oral or intravenous use of traditional Chinese medicine therapies containing astragalus and other ingredients along with conventional chemotherapy improves quality of life scores and increases the rate of tumor response by 52% when compared with chemotherapy alone (102099). It is unclear if these effects are due to astragalus, the other ingredients, or the combination.
• Congestive heart failure (CHF). It is unclear if oral or intravenous astragalus is beneficial for CHF. Details: One preliminary clinical study in adults with CHF shows that intravenous infusion of astragalus 60 grams daily for 20 days improves left ventricular ejection fraction (LVEF) and left ventricular end-diastolic volume (LVEDV) when compared with conventional treatment (32755). However, another preliminary clinical study shows that intravenous astragalus 60 grams daily for 28 days in combination with conventional treatment for CHF does not improve LVEF, LVEDV, or left ventricular end-systolic volume when compared with conventional treatment alone (32812). Both of these studies have methodological problems. Other preliminary clinical research shows that taking oral astragalus 2.25-7.5 grams twice daily for 14-30 days in combination with conventional treatment improves cardiac function, LVEF, and walking distance when compared with conventional treatment alone (33018,33022).
• Diabetes. It is unclear if oral or intravenous astragalus is beneficial for diabetes. Details: A meta-analysis of preliminary clinical research in Chinese patients with type 2 diabetes shows that astragalus, given as a daily intravenous injection 40-80 grams or taken orally 40-60 grams daily as an aqueous decoction for up to 4 months, modestly improves fasting and postprandial blood glucose levels when compared to a control group. Oral astragalus seems to improve these outcomes slightly more than intravenous astragalus. Orally, astragalus also seems to improve fasting insulin levels and insulin resistance, although its effect on glycated hemoglobin (HbA1c) is inconsistent. (95909). Astragalus has also been evaluated as an adjuvant therapy to metformin. A meta-analysis of several mostly small clinical studies in adults with type 2 diabetes shows that taking astragalus 10-50 grams daily with metformin modestly reduces fasting blood glucose by approximately 12 mg/dL, 2-hour postprandial blood glucose by 12 mg/dL, and HbA1c by 0.9% when compared with metformin monotherapy (112809). However, the clinical studies included in both of these meta-analyses are of low methodological quality and include only Chinese patients, which limits the reliability and generalizability of the results. Some research has evaluated astragalus in combination with other ingredients. One preliminary clinical study in adults with previously untreated type 2 diabetes shows that taking astragalus 210 mg, goldthread 350 mg, and honeysuckle 840 mg three times daily before meals for 3 months does not improve fasting blood glucose, postprandial blood glucose, or HbA1c when compared with placebo, although glucose disposal rate is modestly improved (32925).
• Diabetic nephropathy. It is unclear if intravenous astragalus is beneficial for diabetic nephropathy. Details: Meta-analyses of small, low-quality clinical studies including over 4700 patients shows that adding intravenous astragalus to routine therapy for 2-12 weeks modestly improves blood urea nitrogen, serum creatinine, creatinine clearance, urinary protein, urinary albumin, and serum albumin when compared with routine therapy alone in patients with diabetic nephropathy (33019,102100).
• Diabetic retinopathy. It is unclear if oral astragalus is beneficial for diabetic retinopathy. Details: A meta-analysis of results from preliminary clinical research suggests that taking astragalus-containing products for up to 10 months modestly improves visual acuity and fundus manifestations when compared with control in patients with diabetic retinopathy. However, the included studies had methodological limitations, and the results were heterogeneous (90655).
• Fibromyalgia. Although there has been interest in using oral astragalus for fibromyalgia, there is insufficient reliable information about the clinical effects of astragalus for this condition.
• Gastric cancer. Oral and intravenous astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of 35 randomized, controlled trials involving 2670 patients with advanced gastric cancer shows that taking either intravenous or oral traditional Chinese medicines containing astragalus in conjunction with platinum-based chemotherapy may improve objective response rate, disease control rate, 1-year survival rate, and quality of life, and may also reduce the severity of chemotherapy-associated adverse effects, when compared with chemotherapy alone (107480). The validity of this meta-analysis is limited by potential publication bias, unclear randomization methods, and substantial heterogeneity among the included studies. Additionally, it is unclear if these effects are due to astragalus, other ingredients, or the combination.
• Hearing loss. It is unclear if intravenous astragalus is beneficial for hearing loss. Details: Preliminary clinical research shows that injecting 0.5 mL/kg of solution containing astragalus 2 grams/mL daily for 10 days improves hearing in people with sudden deafness or acute acoustic trauma when compared with a control group (33028,33033).
• Hepatitis B. Although there has been interest in using oral astragalus for hepatitis B, there is insufficient reliable information about the clinical effects of astragalus for this condition.
• HIV/AIDS. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: One small clinical study shows that taking a combination containing mainly Baikal skullcap root, glossy privet fruit, astragalus root, and Eupolyphaga et polyphage (Ailing granules) 20 grams twice daily for 4 months improves HIV/AIDS symptoms and CD4 cell counts (32824). However, taking a different combination containing licorice, yin chen, white mulberry, astragalus, and safflower orally for 12 weeks is associated with only a slight decline in viral load and no change in CD4 cell counts (32795). The effects of astragalus alone are unclear.
• IgA nephropathy. Although there has been interest in using intravenous astragalus for IgA nephropathy, there is insufficient reliable information about the clinical effects of oral or intravenous astragalus for this condition.
• Lung cancer. Early research suggests that oral or intravenous astragalus seems to increase the effectiveness of platinum-based chemotherapy or radiotherapy in patients with advanced non-small cell lung cancer (NSCLC). Details: One meta-analysis in patients with advanced NSCLC shows that intravenous administration of astragalus along with platinum-based chemotherapy increases the rate of one-year survival by 40% when compared with platinum-based chemotherapy alone (100698). Other meta-analyses in these patients show that use of oral or intravenous astragalus-containing herbal products appears to reduce the risk of death at six months by 42%, at one year by 33%, at two years by 27% to 33%, and at three years by 15% to 24% when compared with platinum-based chemotherapy or radiotherapy alone (15001,90656). However, most studies included in the meta-analyses were low-quality, and all studies were conducted in China. Higher quality studies in other geographic locations are needed to confirm these findings.
• Membranous nephropathy. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A meta-analysis of 48 small, low-quality clinical studies in adults with idiopathic membranous nephropathy conducted in China shows that taking astragalus 20-90 grams per day in combination with a variety of other supplements, in addition to conventional therapy, modestly improves the rate of complete or partial renal remission when compared with conventional therapy (e.g. angiotensin-converting enzyme inhibitors, immunosuppression). Adding astragalus combinations to conventional therapy also modestly reduces proteinuria and serum creatinine (112010). However, the interpretation of these findings is limited by the heterogeneity of the included studies and variable definitions of complete and partial renal remission. Additionally, it is unclear if these effects are due to astragalus, other ingredients, or the combination.
• Menopausal symptoms. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: One preliminary clinical study shows that taking Dang Gui Buxue Tang, an herbal decoction containing astragalus and dong quai extract, 3 grams daily for 6 months does not reduce the incidence of menopausal hot flashes (32857). However, another preliminary clinical study shows that taking Dang Gui Buxue Tang 3-6 grams daily for 12 weeks reduces hot flash frequency by up to 40% and night sweat frequency up to 53% when compared to baseline (90659). Each 1.5 grams of Dang gui Buxue Tang contains extract prepared from dong quai 1.5 grams and astragalus 7.5 grams (90659). Another small clinical study in females with moderate or severe menopausal symptoms shows that taking a tablet containing astragalus and Rubus coreanus extract 1000 mg twice daily for 12 weeks modestly improves symptom and quality of life scores when compared with placebo (110478). However, the validity of these findings is limited by a high rate of study participant discontinuation.
• Myocardial infarction (MI). Although there has been interest in using intravenous astragalus for MI, there is insufficient reliable information about the clinical effects of oral or intravenous astragalus for this purpose.
• Myocarditis. It is unclear if oral or injectable astragalus is beneficial for viral myocarditis. Details: A meta-analysis of 13 mostly small, low-quality clinical studies in adults and children with viral myocarditis shows that astragalus injection may improve the total effective rate and reduce markers of inflammation when compared with conventional treatment (110477). However, most studies included in the meta-analysis were low-quality, dosing was variable, and all studies were conducted in China. Higher quality studies in other geographic locations are needed to confirm these findings. Several clinical studies report improvements in cardiac enzymes, cardiac function, or viral load in peripheral leukocytes with the use of astragalus preparations, but results are inconsistent and the methodological quality of these studies is low (33084,33105,33217,33218,33220,33221,33223). Doses demonstrating benefit include intramuscular astragalus extract, equivalent to 8 grams daily for 3-4 months (33105) and intravenous astragalus extract 40 grams daily for 2 weeks (33218,33221). It is unclear if astragalus speeds recovery or reduces the risk of mortality due to myocarditis.
• Nephrotic syndrome. It is unclear if oral astragalus is beneficial for preventing infections in patients with nephrotic syndrome. Details: A meta-analysis of results from two preliminary clinical studies shows that taking astragalus 7.5-15 grams twice daily for 3-6 months reduces the risk of infection by 38% when compared to control in children with nephrotic syndrome (33036).
• Obesity. Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Preliminary clinical research shows that taking a combination of astragalus, rhubarb, turmeric, red sage root, ginger, and gallic acid concurrently with a reduced-calorie diet does not improve weight loss when compared with a reduced-calorie diet alone in adults who are overweight or obese (20347).
• Post-stroke fatigue. It is unclear if oral astragalus is beneficial for post-stroke fatigue. Details: A preliminary clinical study shows that taking astragalus extract (Sun Ten Pharmaceutical Co. Ltd.) 2.8 grams three times daily for 28 days improves fatigue and some quality of life measures such as social functioning, role functioning, and physical functioning, when compared with placebo in patients experiencing fatigue following a recent stroke (95908).
• Systemic lupus erythematosus (SLE). It is unclear if intravenous astragalus is beneficial for lupus nephritis. There is insufficient reliable information about the clinical effects of oral astragalus for this condition. Details: A preliminary clinical study in adults with lupus nephritis shows that intravenous infusions of astragalus 40 grams daily for 12 consecutive days monthly for 3 months, in combination with corticosteroids and cyclophosphamide once monthly as an intravenous drip, can improve symptoms, reduce infections, and reduce urinary protein excretion when compared to treatment with corticosteroids and cyclophosphamide alone (32838).
• Tinnitus. It is unclear if intravenous astragalus is beneficial for tinnitus. Details: Preliminary clinical research shows that injecting 0.5 mL/kg of solution containing astragalus 2 grams/mL daily for 10 days reduces tinnitus in patients with acute acoustic trauma when compared to a control group (33028).
• Upper respiratory tract infection (URTI). Although there has been interest in using oral astragalus for URTI prevention, there is insufficient reliable information about the clinical effects of astragalus for this purpose.
• Urinary Tract Infections (UTIs). Oral astragalus has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A moderate-size clinical study in females aged 18-45 with uncomplicated UTIs shows that taking a supplement containing astragalus root extract 100 mg, d-mannose, citric acid, dandelion, and prebiotics as a sachet dissolved in water twice daily for 5 days improves complete resolution of UTI symptoms and clearance of bacteriuria at days 6 and 35 when compared with placebo (111757). However, it is unclear if these effects are due to astragalus, other ingredients, or the combination.
• Wound healing. Although there has been interest in using topical astragalus for wound healing, there is insufficient reliable information about the clinical effects of astragalus for this purpose. More evidence is needed to rate astragalus for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Abdominal pain. Although there has been interest in using oral atractylodes for abdominal pain, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Anorexia nervosa. Although there has been interest in using oral atractylodes for anorexia nervosa, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Cachexia. It is unclear if oral atractylodes is beneficial in patients with cachexia. Details: Preliminary clinical research in people with gastric cancer-related cachexia shows that taking an isolated constituent of atractylodes, atractylenolide I, 1.32 grams orally daily for 7 weeks moderately improves appetite scores and functional ability scores when compared with fish oil 0.45 grams (15706).
• Common cold. Although there has been interest in using oral atractylodes for the common cold, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Diarrhea. Although there has been interest in using oral atractylodes for diarrhea, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Gastroesophageal reflux disease (GERD). Although there has been interest in using oral atractylodes for GERD, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Influenza. Although there has been interest in using oral atractylodes for Influenza, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Intracranial hemorrhage. Atractylodes has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in adults with chronic subdural hematoma (CSDH) who underwent burr hole surgery shows that taking atractylodes rhizome as part of traditional Japanese medicine products also containing Asian water plantain, mushrooms, and cinnamon for 3 months reduces postoperative recurrence of CSDH when compared with usual care. Subgroup analysis suggests the combination treatment containing atractylodes may be most effective for the acute on chronic CSDH subtype (112827). It is unclear whether these effects are due to atractylodes, other ingredients, or the combination.
• Lung cancer. Although there has been interest in using oral atractylodes for lung cancer, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Obesity. Although there has been interest in using oral atractylodes for obesity, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Peritoneal dialysis. Although there has been interest in using oral atractylodes for peritoneal dialysis, there is insufficient reliable information about the clinical effects of atractylodes for this purpose.
• Rheumatoid arthritis (RA). Although there has been interest in using oral atractylodes for RA, there is insufficient reliable information about the clinical effects of atractylodes for this purpose. More evidence is needed to rate atractylodes for these uses.

LIKELY EFFECTIVE

• Mental alertness. Consumption of black tea can improve or prevent declines in mental alertness, attention, and cognitive capacity. Details: Preliminary clinical research in healthy people shows that consumption of black tea containing caffeine 30-100 mg, as well as other caffeinated beverages, throughout the day prevents a decline in alertness and cognitive capacity when compared with placebo (4221,4224,92404). Also, some preliminary clinical research in healthy people shows black tea can improve attention. The effect may be dose-dependent, with greater effects observed in patients consuming black tea that contains higher concentrations of caffeine and theanine (92404). Caffeine can improve mental performance and alertness following prolonged sleep deprivation (10205). Combining caffeine with taurine seems to provide a small improvement in mental performance (9899). Combining caffeine with glucose as an "energy drink" seems to improve mental performance when compared with placebo or either caffeine or glucose alone (13732).

POSSIBLY EFFECTIVE

• Hypotension. Consumption of black tea seems to increase blood pressure in older patients with postprandial hypotension. Details: Several clinical studies in older adults with postprandial hypotension show that drinking caffeinated beverages such as black tea seems to increase blood pressure in the short-term when compared with placebo or not drinking caffeinated beverages (11834,11835).
• Myocardial infarction (MI). Consumption of black tea seems to be associated with a reduced risk of MI. Details: There is some evidence that people who consume at least one cup of black tea daily have a lower risk of MI when compared with not drinking tea (4222,8119,8121). Consuming black tea for at least one year before a heart attack also seems to be associated with a 28% to 44% reduced risk of subsequent death when compared with not drinking tea (8120).
• Osteoporosis. Consumption of black tea seems to have beneficial effects on bone health. Details: Population research suggests that consuming black tea is associated with increased bone mineral density, especially in older females aged 65-76 years and those whose habitual tea consumption extends beyond 10 years, when compared with not drinking tea (6404,8116). Black tea consumption also seems to be associated with a lower risk of hip fracture in adults aged 50 years or more (10949,15014).
• Ovarian cancer. Consumption of black tea seems to be associated with a reduced risk of ovarian cancer. Details: Adults who regularly consume tea, including black tea or green tea, appear to have a significantly lower risk of developing ovarian cancer when compared with those who never or seldom consume tea (9228,13208). In one prospective population study, adults who consume at least 2 cups of tea daily have a 46% lower risk of ovarian cancer when compared with those who do not regularly consume tea (13208).
• Parkinson disease. Consumption of black tea seems to be associated with a reduced risk of Parkinson disease.
• Parkinson disease. For males, the effects seem to be dose related. For example, males consuming 421-2716 mg of total caffeine (equivalent to 5-33 cups of black tea) from any source daily seem to have the greatest reduction in risk. However, there seems to be a significant reduction in risk even with consumption of as little as 124-208 mg of total caffeine (equivalent to 1-3 cups of black tea) daily (6022). In females, the effects do not seem to be dose related. Moderate consumption of caffeine (equivalent to 1-4 cups black tea daily) seems to provide the most reduction in risk (1238). Also, drinking tea appears to be associated with a reduced incidence of Parkinson disease among people who smoke when compared with almost never drinking tea (9236).

POSSIBLY INEFFECTIVE

• Bladder cancer. Consumption of black tea does not seem to be associated with a reduced risk of urinary tract cancers, including bladder cancer. Details: Observational research has found that drinking tea, including black and green tea, is not associated with a reduced risk of urinary tract cancers, including bladder cancer, when compared with those who almost never drink black tea (36482,36561).
• Breast cancer. Consumption of black tea does not seem to be associated with a reduced risk of breast cancer. Details: Observational research has found that drinking black tea is not associated with a lower risk of breast cancer when compared with not drinking black tea (9235,92406).
• Colorectal cancer. Consumption of black tea does not seem to be associated with a reduced risk of colorectal cancer. Details: Some population research has found that drinking tea, including black tea or green tea, might be associated with a reduced risk of colorectal cancer when compared with little or no tea consumption (9234,36535). However, most research has found that drinking tea is not associated with a reduced risk of colorectal cancer (9222,9223,9234,36385,36416,36474,36535,36553,36582). Additionally, some population research has found that increased consumption of black tea is associated with an increased risk of colorectal cancer when compared with those who almost never drink black tea (8041,36482).
• Diabetes. Consumption of black tea does not seem to be associated with a reduced risk of diabetes and does not seem to improve glycemic control in patients with diabetes. Details: Preliminary clinical research in patients with diabetes shows that taking an extract of black and green tea daily for 3 months does not reduce glycated hemoglobin (HbA1C) when compared with placebo (36434). Additionally, population research in Japanese adults has found that drinking at least 1 cup of black tea daily does not seem to be associated with a lower risk of developing type 2 diabetes when compared with drinking less than 1 cup daily (14313).
• Endometrial cancer. Consumption of black tea does not seem to be associated with a reduced risk of endometrial cancer. Details: A meta-analysis of results from population research has found that increasing black tea intake by 1 cup per day is not associated with a reduced risk of endometrial cancer when compare with no or low black tea consumption (92409).
• Esophageal cancer. Consumption of black tea does not seem to be associated with a reduced risk of esophageal cancer. Details: A meta-analysis of results from population research has found that people who drink higher amounts of black tea do not have a reduced risk of esophageal cancer when compared with those who drink lesser amounts (90186). However, some population research has found that drinking large amounts of strong black tea is associated with a two-fold INCREASED risk for esophageal cancer when compared with people who drink less black tea (102756).
• Gastric cancer. Consumption of black tea does not seem to be associated with a reduced risk of gastric cancer. Details: While some epidemiological evidence has found that increased consumption of tea, including black or green tea, is associated with a reduced risk of stomach cancer (36561), most evidence shows that drinking tea is not associated with a reduced stomach cancer risk (9222,9223,36553,36333). Additionally, some epidemiological evidence has found that consumption of tea may be associated with an increased risk of mortality due to stomach cancer when compared with nonregular tea consumption (36474).
• Lung cancer. Consumption of black tea does not seem to be associated with a reduced risk of lung cancer. Details: Most population research found that drinking up to 5 cups of black tea daily is not associated with reduced risk of lung cancer (36474,36553,90177,97128). One population study even found an increase in risk of lung cancer associated with tea consumption, including black and green tea, when compared with nonregular tea consumption. However, this study did not adjust for the patients' baseline smoking status and carcinogen exposure (36474).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Atherosclerosis. Consumption of black tea seems to be associated with a reduced risk of severe, but not mild or moderate, aortic atherosclerosis. Details: Epidemiological research has found that increased intake of brewed black tea (125-500 mL or 1-4 cups) is associated with a reduced risk of severe aortic atherosclerosis, especially in females. However, black tea intake is not associated with a reduced risk of mild or moderate aortic atherosclerosis (3450).
• Cardiovascular disease (CVD). Consumption of black tea may be associated with a modest reduction in CVD-related mortality. Details: A prospective cohort study of nearly 500,000 adults 40-69 years of age in the United Kingdom Biobank shows that black tea intake in quantities greater than 1 cup daily is associated with a modestly reduced rate of CVD-related mortality over a mean follow-up of about 11 years. Consuming anywhere between 2-9 cups of black tea daily resulted in similar hazard ratios; consuming 10 or more cups per day may be associated with a further reduction in risk (108970).
• Chronic obstructive pulmonary disease (COPD). It is unclear if black tea is associated with a reduced risk of COPD. Details: One prospective, observational study in older adults has found that consumption of at least 3 cups daily of tea, including black tea, oolong tea, and green tea, is associated with a 23% reduced risk of COPD prevalence and a 65% reduced risk of developing COPD in models adjusted for potential confounders when compared with the lowest tea consumption (107201).
• Coronary heart disease (CHD). It is unclear if black tea is associated with a reduced risk of CHD or CHD-related mortality. Details: Although a meta-analysis of population research found that general tea consumption might protect against cardiovascular disease (CVD) or death from CVD, a sub-group analysis has found that drinking black tea does not seem to offer protection (102728). In fact, some individual population research found that black tea consumption is associated with an increased incidence of CHD progression and CHD mortality in those with existing CHD (220,36339). In contrast, observational research in healthy adults has found that those who drink black tea regularly have a decreased risk of developing CHD (7726,36363,36391,36420).
• Dementia. It is unclear if black tea, consumed in conjunction with coffee, is associated with a reduced risk of dementia. Details: A prospective, observational study suggests consumption of tea, including black tea, of at least 4 cups with 0.5-1 cups of coffee daily is associated with a 30% reduced risk of dementia when compared with not drinking tea or coffee. Also, drinking 2-3 cups daily of both tea and coffee is associated with a 28% reduced risk of dementia stroke when compared with not drinking tea and coffee (107204).
• Depression. It is unclear if black tea is associated with a reduced risk of depression. Details: Observational research in healthy adult office workers without a history of anxiety or depression has found that consumption of at least 1-4 cups daily of black tea, as well as caffeine from other sources, is associated with a 41% to 61% reduced risk of depression scores in multiple models adjusted for potential confounders (107202).
• Diarrhea. Black tea seems to improve stool consistency in children with acute nonbacterial diarrhea. Details: Preliminary clinical research in children with acute nonbacterial diarrhea shows that taking a one-time dose of black tea 500-1000 mg as tablets dissolved in water improves stool consistency after 24 hours when compared with placebo (97126).
• Halitosis. It is unclear if black tea is beneficial in patients with halitosis. Details: A very small study in healthy adults shows that rinsing with 10 mL of black tea for 30 seconds following L-cysteine-induced halitosis improves the concentration of volatile sulfur compounds in breath samples when compared with baseline. These concentrations also were improved at 2 hours, but not at 1 hour, when compared with control. It is unclear if the improvement from baseline differed between groups (107203).
• Headache. Although there has been interest in consuming black tea for headache, there is insufficient reliable information about the clinical effects of black tea for this purpose.
• Hyperlipidemia. The evidence for the benefits of black tea in reducing cholesterol is conflicting. Details: One small clinical study in mildly hypercholesterolemic adults shows that drinking black tea reduces total cholesterol and low-density lipoprotein (LDL) cholesterol levels (36401). However, other studies have found that drinking black tea or taking black tea extract does not reduce cholesterol or triglyceride levels in healthy people or in those with cardiovascular risk factors (36376,36436,36581,36592). Meta-analyses of clinical research also report conflicting findings. Several meta-analyses in healthy people and in patients with coronary artery disease or hypercholesterolemia suggest that drinking black tea or taking black tea extract does not reduce total cholesterol or LDL cholesterol levels when compared with placebo (92407,102738). However, several other small meta-analyses of clinical studies in healthy people or those at risk for cardiovascular disease show that drinking black tea 3-5 servings daily or taking black tea extract 1-6 grams daily for up to 6 months modestly reduces LDL cholesterol levels when compared with placebo (90146,92408). Reasons for the conflicting findings are not entirely clear. One analysis suggests that black tea has a greater effect on LDL cholesterol in patients with higher cardiovascular risks than those who are healthy (92408). The effect of black tea on cholesterol levels does not appear to be influenced by the duration of treatment or the type of black tea used (tea leaf vs. tea extract) (92407,92408).
• Hypertension. Consumption of black tea seems to modestly reduce blood pressure, although any effect may not be considered clinically significant. Details: Most epidemiological research and some meta-analyses of clinical research show that consumption of 3-5 cups of black tea daily or black tea extracts 6 grams daily for up to 6 months can modestly reduce blood pressure, although any benefits are not likely to be considered clinically significant (36400,90146,92405,104582,104583). One meta-analysis of clinical research in a mixed population with normotension, hypertension, coronary artery disease, mild hypercholesterolemia, or at high risk of cardiovascular disease shows that taking black tea 300-1800 mL daily for 7 days to 6 months reduces systolic blood pressure by about 1.5 mmHg and diastolic blood pressure by about 0.8 mmHg when compared with control (104582). Other analyses show similar reductions of up to 2 mmHg (90146,92405). However, the effect in patients with hypertension is unclear because this population is rarely analyzed separately. In a meta-analysis of three clinical trials limited to patients with hypertension, and excluding studies related to purified black tea extracts or isolated components, there was no statistically significant effect on blood pressure when compared with control (104583). Interestingly, most of the individual clinical studies in these analyses did NOT find a reduction in blood pressure from black tea consumption (104582). This might be because most of these studies had a short duration (1452,36376,36581,104583) or were underpowered to detect a modest effect (36436). Furthermore, these studies did not use out-of-office blood pressure monitoring, such as 24-hour ambulatory blood pressure monitoring (ABPM). A robust clinical study using ABPM in patients with normotension and hypertension shows that drinking 3 cups of black tea (1493 mg powdered black tea solids containing 429 mg of polyphenols) daily for 6 months results in about 2 mmHg reduction in both systolic and diastolic blood pressure when compared with placebo (97127).
• Kidney stones (nephrolithiasis). Consumption of black tea seems to reduce the risk of developing kidney stones. Details: Population research has found that consuming one 8 oz cup of tea, such as black tea, daily is associated with an 8% decreased risk of developing kidney stones in females (4216).
• Oral cancer. Although there has been interest in consuming black tea for oral cancer, there is insufficient reliable information about the clinical effects of black tea for this purpose.
• Overall mortality. Consuming black tea may be associated with a modest reduction in overall mortality. Details: A prospective cohort study of nearly 500,000 adults between 40-69 years of age in the United Kingdom Biobank shows that consuming black tea in quantities greater than 1 cup daily is associated with a modestly reduced all-cause mortality over a mean follow-up of about 11 years. Increasing consumption beyond 2-3 cups per day does not further reduce overall mortality rate (108970).
• Pancreatic cancer. It is unclear if consumption of black tea is associated with a reduced risk of pancreatic cancer. Details: One prospective, observational study has found that Japanese males who drink more than 1 cup of black tea daily are not at a lower risk of developing pancreatic cancer when compared with those who almost never drink black tea (36482). However, another observational study in Polish males has found that increased tea consumption over a lifetime is associated with a reduced risk of pancreatic cancer (36550).
• Prostate cancer. It is unclear if consumption of black tea is beneficial for reducing the risk of prostate cancer. Details: Observational research has found that black tea consumption is associated with a reduced risk of prostate cancer when compared with those who almost never drink black tea (36482).
• Renal cell carcinoma. It is unclear if consumption of black tea is beneficial for reducing the risk of kidney cancer. Details: Prospective, observational research in males has found that drinking higher amounts of tea, including black and green tea, is associated with an increased risk of kidney cancer when compared with nonregular tea consumption (36474).
• Stress. It is unclear if consumption of black tea is beneficial for reducing stress. Details: Preliminary clinical research in healthy, nonsmoking males shows that drinking black tea daily for 6 weeks does not improve blood pressure, heart rate, or subjective stress ratings in response to stressful tasks when compared with placebo (36426).
• Stroke. It is unclear if consumption of black tea is associated with a reduced risk of stroke or death from stroke. Details: Several prospective, observational studies have found that higher intake of tea, including black tea, is associated with a decreased incidence of stroke when compared with lower intake (7716,107204), as well as a modestly reduced risk of stroke-related mortality (108970). Consumption of at least 2 cups daily of tea, including black tea, is associated with a 16% reduced risk of stroke when compared with drinking no tea. Drinking 2-3 cups daily of both tea and coffee is associated with a 38% reduced risk of stroke when compared with not drinking either of these beverages. The combination of 0.5-1 cups of coffee and 2-3 cups of tea daily, but not tea alone, also is associated with a 50% reduced risk of post-stroke dementia when compared with not drinking tea and coffee (107204). More evidence is needed to rate black tea for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Mental alertness. One preliminary clinical study shows that inhaling the aroma of jasmine essential oil while taking a visual vigilance test does not improve reaction time or accuracy when compared with placebo (57019). Another preliminary study in healthy male students shows that inhaling the aroma of jasmine essential oil for 30 minutes between concentration tests does not reduce mental fatigue when compared with placebo (56977). More evidence is needed to rate jasmine for this use.

POSSIBLY EFFECTIVE

• Burns. Topical cream containing sea buckthorn seems to improve burn healing. Details: Clinical research in patients with second-degree burn injuries shows that topical application of sea buckthorn 40% cream once daily reduces healing time by about 4.5 days when compared with a specific cream containing silver sulfadiazine 1% (Flamexin, Sina Daru) (106091).

POSSIBLY INEFFECTIVE

• Atopic dermatitis (eczema). Neither oral nor topical sea buckthorn products seem to be beneficial for atopic dermatitis. Details: Preliminary clinical research shows that taking sea buckthorn seed and pulp oils 5 grams daily orally for 4 months may improve the severity of atopic dermatitis when compared to pretreatment; however, treatment with the same dose of sea buckthorn seed oil does not have this effect, and the effect of sea buckthorn pulp oil does not appear to be significantly different than placebo (74021). Additionally, applying sea buckthorn 10% or 20% cream topically for 4 weeks does not seem to reduce the severity of atopic dermatitis when compared with placebo in patients with mild-to-moderate atopic dermatitis (74091).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Acne. Although there is interest in using topical sea buckthorn for acne, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Aging skin. Although there is interest in using oral or topical sea buckthorn for aging skin, there is insufficient reliable information about the clinical effects of sea buckthorn for this purpose.
• Asthma. Although there is interest in using oral sea buckthorn for asthma, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Cancer. Although there is interest in using oral sea buckthorn for reducing morbidity related to cancer and side effects related to chemotherapy, there is insufficient reliable information about the clinical effects of sea buckthorn for these uses.
• Cirrhosis. It is unclear if oral sea buckthorn is beneficial for patients with cirrhosis. Details: Preliminary clinical research shows that taking sea buckthorn extract 15 grams three times daily for 6 months might reduce liver enzymes and markers of liver inflammation in patients with cirrhosis when compared with a control group taking vitamin B complex (16694).
• Common cold. It is unclear if oral sea buckthorn is beneficial for preventing or treating the common cold. Details: Clinical research in healthy volunteers shows that consuming sea buckthorn berries 28 grams daily in frozen puree for 90 days does not reduce the risk of developing the common cold or reduce the duration of symptoms of the common cold when compared with placebo (16692).
• Diabetes. Although there is interest in using oral sea buckthorn for diabetes, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Dry eye. It is unclear if oral or topical sea buckthorn is beneficial for dry eye. Details: Preliminary clinical research in adults with dry eye shows that taking a specific sea buckthorn seed and pulp oil product (Omega-7, Aromtech Ltd.) 1 gram orally twice daily for 3 months reduces subjective assessments of eye redness and burning and reduces tear film osmolarity when compared with placebo (17406,74069). Preliminary clinical research also shows that using an eyelid spray containing sea buckthorn seed oil 0.4% and sodium hyaluronate 0.02% (Aromtech Ltd., and Finnsusp Ltd.) four times daily for 1.5 months reduces symptoms of dryness and watering by a small amount when compared to a control spray. However, there was no effect on feelings of grittiness, burning, redness, or other symptoms (102845).
• Dry skin. Although there is interest in using topical sea buckthorn for dry skin, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Dyslipidemia. It is unclear if oral sea buckthorn is beneficial for hyperlipidemia. Details: Some clinical research shows that sea buckthorn berries and extracts can improve the lipid profile in people with hyperlipidemia and/or nonalcoholic fatty liver disease (NAFLD). In a meta-analysis of 11 studies from China and Finland, consuming sea buckthorn berries or extracts for between 12 days and 4 months decreased total cholesterol and low-density lipoprotein (LDL) cholesterol by about 24 mg/dL and triglycerides by about 41 mg/dL and increased high-density lipoprotein (HDL) cholesterol by about 10 mg/dL when compared with baseline or a control group (96555). The sea buckthorn doses and formulations used in these studies were not identified. Additionally, a very small clinical study in females aged 50-61 with hypercholesterolemia shows that drinking sea buckhorn juice (ZAMIO, Ltd) 100% 50 mL daily for 8 weeks modestly improves HDL by about 2 mg/dL and LDL by 10 mg/dL, but does not impact total cholesterol or triglycerides when compared to baseline (112171). However, the validity of these findings is limited by the lack of a comparator group. In contrast, clinical research in patients with hypercholesterolemia shows that taking sea buckthorn puree 30 grams three times daily for 90 days does not improve lipid markers or blood pressure when compared with placebo (106090).
• Dyspepsia. It is unclear if oral sea buckthorn is beneficial for dyspepsia. Details: Preliminary clinical research shows that taking sea buckthorn dry emulsion (prepared with the fruit juice and oil) daily for 8 weeks increases levels of the appetite regulators leptin and neuropeptide Y, which tend to be lower in children with functional dyspepsia; however, sea buckthorn does not appear to improve gastric emptying rates in these patients. Doses were administered according to age: 2.5 grams twice daily for children aged 1-3 years, 5 grams twice daily for children aged 4-6 years old, and 7.5 grams twice daily for children aged 7 years (94800).
• Gout. Although there is interest in using oral sea buckthorn for gout, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Hypertension. It is unclear if oral sea buckthorn is beneficial for hypertension. Details: A meta-analysis of preliminary clinical research in patients with essential hypertension shows that taking sea buckthorn flavones 10-30 mg three times daily for 1-8 months reduces systolic and diastolic blood pressure to a similar extent as calcium channel blockers and angiotensin converting enzyme (ACE) inhibitors (74090).
• Impaired glucose tolerance (prediabetes). It is unclear if oral sea buckthorn is beneficial for prediabetes. Details: A small clinical trial in patients with impaired glucose tolerance shows that taking sea buckthorn fruit puree 90 mL daily in three divided doses for 5 weeks modestly reduces fasting levels of blood glucose when compared with taking placebo. There was no effect on postprandial glucose levels or blood pressure (106088).
• Kidney failure. It is unclear if oral sea buckthorn oil is beneficial for patients on hemodialysis. Details: Preliminary clinical research shows that taking sea buckthorn oil 2 grams daily for 8 weeks does not improve levels of urea, creatinine, or calcium, reduce markers of inflammation, or improve salivary flow rate when compared with placebo in patients with chronic kidney disease (CKD) who are receiving hemodialysis (94798).
• Melasma. Although there is interest in using topical sea buckthorn for melasma, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Nephrotic syndrome. It is unclear if oral sea buckthorn is beneficial for nephrotic syndrome. Details: Preliminary clinical research in patients with idiopathic nephrotic syndrome shows that taking hydroalcoholic extract of sea buckthorn 350 mg orally twice daily for 3 months in addition to standard therapy reduces edema, anorexia, and the extent of proteinuria when compared to baseline. It is unclear if the effects of sea buckthorn plus standard therapy are significantly better than the effects of standard therapy alone (94799).
• Obesity. It is unclear if oral sea buckthorn is beneficial for obesity. Details: Preliminary clinical research shows that taking sea buckthorn berries, berry oil, or phenolic alcoholic extract does not reduce body weight in overweight or obese females, although it seems to reduce low-density lipoprotein (LDL) cholesterol (35463,94796).
• Peptic ulcers. Although there is interest in using oral sea buckthorn for peptic ulcers, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Psoriasis. It is unclear if topical sea buckthorn is beneficial for psoriasis. Details: One small clinical study in patients with previously untreated psoriasis shows that topically applying an unknown amount of sea buckthorn fruit extract suspended in corn oil twice daily for 8 weeks reduces the severity of psoriasis lesions when compared with corn oil alone (102844).
• Radiation dermatitis. Although there is interest in using topical sea buckthorn for radiation dermatitis, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Rheumatoid arthritis (RA). Although there is interest in using oral sea buckthorn for RA, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Sunburn. Although there is interest in using topical sea buckthorn for sunburn prevention, there is insufficient reliable information about the clinical effects of sea buckthorn for this condition.
• Vaginal atrophy. It is unclear if oral or intravaginal sea buckthorn is beneficial for vaginal atrophy. Details: Preliminary clinical research shows that taking 3 grams of sea buckthorn oil daily for 3 months does not improve vaginal dryness, itching, burning, pH, or elasticity when compared with placebo in postmenopausal individuals with vaginal atrophy (94797). Topical sea buckthorn oil has also been evaluated, in combination with other ingredients. Clinical research in adults with postmenopausal vulvovaginal atrophy shows that intravaginal application of a specific gel (Meclon Idra Alfasigma, Giellepi) containing sea buckthorn oil, aloe vera, 18-beta-glycyrrhetic acid, hyaluronic acid, and glycogen modestly improves sexual function and reduces vaginal pain, dyspareunia, and symptoms such as vaginal dryness, itching, and burning when compared with a placebo gel. The gel was used daily for 14 days and then twice weekly for 60 days after a 2-week washout (106089). It is unclear if these benefits are due to sea buckthorn oil, other ingredients, or the combination.
• Wound healing. Although there is interest in using topical sea buckthorn for healing of pressure ulcers, skin ulcers, or other wounds, there is insufficient reliable information about the clinical effects of sea buckthorn for this purpose.
• Wrinkled skin. Oral sea buckthorn oil has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Preliminary clinical research in people with facial photodamage and severe fine wrinkling shows that taking a specific combination product (GliSODin Skin Nutrients Advanced Anti-Aging Formula, Isocell North America Inc.) as one capsule orally three times daily for 90 days, in addition to applying tazarotene 0.1% cream, improves the appearance of wrinkles, hyperpigmentation, and photodamage when compared with using tazarotene 0.1% cream alone. Three capsules of this product contain sea buckthorn berry oil 200 mg, krill oil 600 mg, zinc 10 mg, vitamin D 400 IU, cacao bean extract 100 mg, hyaluronic acid 30 mg, red clover flower isoflavones 30 mg, and a specific proprietary ingredient called GliSODin 500 mg (91778). It is not clear if these effects are due to sea buckthorn, the other ingredients, or the combination. More evidence is needed to rate sea buckthorn for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• HIV/AIDS. Preliminary clinical research in HIV patients using antiretroviral therapy shows that taking a specific product containing sorghum extract and other ingredients (Jobelyn, Health Forever Product Ltd.) orally twice daily for 12 weeks improves CD4+ T cell count when compared with antiretroviral therapy alone. Hemoglobin levels were increased compared with baseline in patients ineligible for antiretrovirals (97674).
• Iron deficiency anemia. Preliminary clinical research shows that taking a specific product containing sorghum extract and other ingredients (Jobelyn, Health Forever Product Ltd.) orally once daily for 3 weeks does not improve red blood cell indices in patients with anemia who were also taking iron 600 mg daily and folic acid 5 mg daily (97675).
• Obesity. Preliminary clinical research in overweight and obese men shows that eating extruded sorghum cereal 40 grams each morning along with a calorie restricted diet for 8 weeks reduces body fat by around 2%, but does not appear to improve body weight, body mass index, or waist circumference, when compared with extruded wheat cereal plus calorie restriction (103898). More evidence is needed to sorghum for these uses.

According to a review by Health Canada, and a subsequent large meta-analysis conducted in the US, drinking up to 4 cups of black tea daily, or approximately 400 mg of caffeine, is not associated with significant adverse cardiovascular, bone, behavioral, or reproductive effects in healthy adults (11733,98806). The US Dietary Guidelines Advisory Committee states that there is strong and consistent evidence that consumption of caffeine 400 mg daily is not associated with increased risk of major chronic diseases, such as cardiovascular disease or cancer, in healthy adults (98806).

POSSIBLY UNSAFE ...when consumed orally long term or in high amounts. Black tea contains a significant amount of caffeine. Chronic use, especially in large amounts, can produce tolerance, habituation, psychological dependence, and other significant adverse effects. Doses of caffeine greater than 600 mg per day, or approximately 6 cups of black tea, have been associated with significant adverse effects such as tachyarrhythmias and sleep disturbances (11832). These effects would not be expected to occur with the consumption of decaffeinated black tea. Keep in mind that only the amount of ADDED caffeine must be stated on product labels. In some studies, consuming amounts over 200 mg daily is associated with a significantly increased risk of miscarriage (16014). This increased risk may be most likely to occur in females with genotypes that confer a slow rate of caffeine metabolism (98806). According to a review by Health Canada, and a subsequent large meta-analysis conducted in the US, most healthy patients can safely consume doses up to 300 mg daily during pregnancy without an increased risk of spontaneous abortion, stillbirth, preterm birth, fetal growth retardation, or congenital malformations (11733,98806). CLOZAPINE (Clozaril) Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = B Theoretically, black tea might increase the levels and adverse effects of clozapine and acutely exacerbate psychotic symptoms.  Details Concomitant administration of black tea and clozapine might theoretically cause acute exacerbation of psychotic symptoms due to the caffeine in black tea. Caffeine can increase the effects and toxicity of clozapine. Caffeine doses of 400-1000 mg daily inhibit clozapine metabolism (5051). Clozapine is metabolized by cytochrome P450 1A2 (CYP1A2). Researchers speculate that caffeine might inhibit CYP1A2. However, there is no reliable evidence that caffeine affects CYP1A2. There is also speculation that genetic factors might make some patients be more sensitive to the interaction between clozapine and caffeine (13741).

However, clinical research suggests that drinking black tea does not significantly affect flurbiprofen plasma levels, metabolism, or elimination (11094).

Mexiletine can decrease caffeine elimination by 50% (1260,11741,24981).

MONOAMINE OXIDASE INHIBITORS (MAOIs) Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = D Theoretically, concomitant use might increase the risk of a hypertensive crisis.  Details Black tea contains caffeine. Caffeine has been shown to inhibit monoamine oxidase (MAO) A and B in laboratory studies (37724,37877,37912,38108). Concomitant intake of large amounts of caffeine with MAOIs might precipitate a hypertensive crisis (15). In a case report, a patient that consumed 10-12 cups of caffeinate coffee and took the MAOI tranylcypromine presented with severe hypertension (91086). Hypertension was resolved after the patients switched to drinking decaffeinated coffee.

NICOTINE Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Probable •  Level of Evidence = B Theoretically, concomitant use might increase the risk of hypertension.  Details Black tea contains caffeine. Concomitant use of caffeine and nicotine has been shown to have additive cardiovascular effects, including increased heart rate and blood pressure. Blood pressure was increased by 10.8/12.4 mmHg when the agents were used concomitantly (36549).

ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = D Theoretically, black tea might reduce the absorption of organic anion-transporting polypeptide (OATP) substrates.  Details In vitro, black tea extract inhibits organic anion-transporting polypeptide (OATP)2B1. OATP2B1 is expressed in the small intestine and liver and is responsible for the uptake of drugs and other compounds. In an animal model, black tea extract was found to inhibit the absorption of rosuvastatin, a substrate of OATP2B1 (104584). However, this effect has not been reported in humans.

PENTOBARBITAL (Nembutal) Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = B Theoretically, black tea might decrease the effects of pentobarbital.  Details Black tea contains caffeine. Theoretically, caffeine might negate the hypnotic effects of pentobarbital (13742).

PHENOTHIAZINES Interaction Rating = Minor Be watchful with this combination. Severity = Mild •  Occurrence = Possible •  Level of Evidence = D Theoretically, phenothiazines might increase the levels and adverse effects of caffeine. Also, black tea may bind to phenothiazines and reduce their absorption.  Details Black tea contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2) (23573,23574). Additionally, the tannins in black tea may bind to phenothiazines in the gastrointestinal tract and reduce their absorption (626,627).

PHENYLPROPANOLAMINE Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Probable •  Level of Evidence = B Theoretically, phenylpropanolamine might increase the risk of hypertension, as well as the levels and adverse effects of caffeine.  Details Black tea contains caffeine. Concomitant use of phenylpropanolamine and caffeine might cause an additive increase in blood pressure (11738). Phenylpropanolamine also seems to increase caffeine serum levels (13743).

PIOGLITAZONE (Actos) Interaction Rating = Moderate Be cautious with this combination. Animal research suggests that caffeine can modestly increase the maximum concentration, area under the curve, and half-life of pioglitazone, and also reduce its clearance. Caffeine and riluzole are both metabolized by cytochrome P450 1A2 (CYP1A2), and concomitant use might reduce metabolism of one or both agents (11739).

ROSUVASTATIN (Crestor) Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = D Theoretically, concomitant use might decrease the levels and clinical effects of rosuvastatin.  Details In animals, taking black tea extract along with rosuvastatin reduces plasma levels of rosuvastatin by approximately 48%. In vitro, black tea extract was found to inhibit organic anion-transporting polypeptide (OATP)2B1, a protein expressed in the small intestine that is responsible for the uptake of rosuvastatin and other compounds (104584). This effect has not been reported in humans.

STIMULANT DRUGS Interaction Rating = Moderate Be cautious with this combination. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects (11832).

TERBINAFINE (Lamisil) Interaction Rating = Minor Be watchful with this combination. TRICYCLIC ANTIDEPRESSANTS (TCAs) Interaction Rating = Minor Be watchful with this combination. Severity = Mild •  Occurrence = Possible •  Level of Evidence = D Theoretically, TCAs might bind with black tea constituents when taken at the same time.  Details In vitro research suggests that the tannins in black tea might cause precipitation of TCAs (626,627). However, this effect has not been reported in humans.

VERAPAMIL (Calan, others) Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = D Consuming large amounts of black tea might decrease the effects of warfarin.  Details In one case, a 67-year-old female who took warfarin and who regularly consumed large amounts of black tea had a stable international normalized ratio (INR) of 1.7 to 2.7. However, the INR increased to 5 when tea consumption was discontinued. It is thought that the vitamin K content of black tea may have reduced the effects of warfarin (16902). Monitor patients carefully who start or discontinue drinking black tea while taking warfarin.

General ...Orally, black tea is well tolerated when consumed as a beverage in moderate amounts.
Most Common Adverse Effects:
Orally: Many of the adverse effects of black tea can be attributed to its caffeine content, such as diuresis, gastric irritation, insomnia, nausea, nervousness, restlessness, tachycardia, tachypnea, tremors, and vomiting.
Serious Adverse Effects (Rare):
Orally: Many of the adverse effects of black tea can be attributed to its caffeine content, such as arrhythmia, chest pain, convulsions, delirium, premature heartbeat, and respiratory alkalosis. Large doses of caffeine can cause massive catecholamine release and subsequent sinus tachycardia, metabolic acidosis, hyperglycemia, and ketosis.

Cardiovascular ...Orally, black tea can cause some cardiovascular-related adverse events. Some of these effects may be due to the caffeine content of black tea. Acute administration of black tea can cause increased blood pressure. However, regular consumption does not seem to increase blood pressure or pulse, even in patients with mild hypertension (1451,1452,2722). Also, epidemiological research suggests that there is no association of caffeine consumption with incidence of hypertension (13739).
Black tea, which contains caffeine, may cause other adverse cardiovascular effects when used orally. These effects include tachycardia, tachypnea, chest pain, premature heartbeat, arrhythmia, and hypertension (2729,11832,11838,13735). Large doses of caffeine can also cause massive catecholamine release and subsequent sinus tachycardia (13734).
There is evidence that daily consumption of strong black tea (2 liters) or black tea solids (4 grams) can raise plasma homocysteine levels. It is unclear if lower doses have this effect (8035). Some epidemiological research has linked tea consumption with ischemic heart disease and total mortality (220,36339). Combining caffeinated beverages such as black tea with ephedra may theoretically increase the risk of adverse cardiovascular events. There is a report of ischemic stroke in an athlete who consumed ephedra 40-60 mg, creatine monohydrate 6 grams, caffeine 400-600 mg, and a variety of other supplements daily for 6 weeks (1275).

Dental ...Orally, black tea may cause tooth surface loss and teeth staining (36370).

Endocrine ...Black tea contains caffeine. Large doses of caffeine can cause massive catecholamine release and subsequent metabolic acidosis, hyperglycemia, and ketosis (13734).
Some evidence shows caffeine is associated with fibrocystic breast disease, breast cancer, and endometriosis. However, other research has not supported this finding (8043). Restricting caffeine in females with fibrocystic breast conditions doesn't seem to affect breast nodularity, swelling, or pain (8996). A population analysis of the Women's Health Initiative observational study found no association between consumption of caffeine-containing beverages such as black tea and the incidence of invasive breast cancer in models adjusted for demographic, lifestyle, and reproductive factors (108806). Also, a dose-response analysis of two low-quality observational studies found that high consumption of caffeine is not associated with an increased risk of breast cancer (108807).

Gastrointestinal ...Orally, caffeine in black tea can cause gastric irritation, nausea, and vomiting (11832,11838,13735). Some believe that long-term use of caffeine can cause withdrawal symptoms following discontinuation of use. However, the existence of caffeine withdrawal is controversial. Some researchers think that if it exists, it appears to be of little clinical significance (11839). Gastrointestinal withdrawal symptoms such as nausea and vomiting have been described. However, these symptoms may be from nonpharmacological factors related to knowledge and expectation of effects. Clinically significant gastrointestinal symptoms caused by caffeine withdrawal may be uncommon (2723,11839).

Hematologic ...Orally, caffeine in black tea can cause hypokalemia (11832,11838,13735). In infants, black tea can cause microcytic anemia (631).

Immunologic ...Orally, caffeine in black tea can cause anaphylaxis in sensitive individuals, although true IgE-mediated caffeine allergy seems to be relatively rare (11315).

Musculoskeletal ...Some epidemiological research suggests that caffeine, which is found in black tea, may be associated with an increased risk of osteoporosis, but conflicting evidence exists. Caffeine can increase urinary excretion of calcium (2669,10202,11317). Females identified with a genetic variant of the vitamin D receptor appear to be at an increased risk for the detrimental effect of caffeine on bone mass (2669). However, moderate caffeine intake, less than 300 mg per day, does not seem to significantly increase osteoporosis risk in most postmenopausal adults with normal calcium intake (2669,6025,10202,11317).
Some researchers believe that stopping regular use of caffeine may cause withdrawal symptoms such as muscle tension and muscle pains. However, these symptoms may be from nonpharmacological factors related to knowledge and expectation of effects (2723,11839).

Neurologic/CNS ...Orally, caffeine in black tea can cause insomnia, nervousness, headache, anxiety, agitation, jitteriness, restlessness, ringing in the ears, tremors, delirium, and convulsions (11832,11838,13735). Caffeine may also exacerbate sleep disturbances in patients with acquired immunodeficiency syndrome (AIDS) (10204).
There is some concern that stopping regular use of caffeine may cause withdrawal symptoms such as headache; tiredness and fatigue; decreased energy, alertness, and attentiveness; drowsiness; decreased contentedness; depressed mood; difficulty concentrating; irritability; and lack of clear-headedness are typical of caffeine withdrawal (13738). Other symptoms such as delirium, nervousness, restlessness, and anxiety have been described. However, these symptoms may be from nonpharmacological factors related to knowledge and expectation of effects (2723,11839).

Oncologic ...There is some evidence that consumption of black tea (greater than 1 cup per day) may increase the risk of colon and rectal cancers (8041,36482). Drinking 3 or more cups daily has been shown to increase the risk of pancreatic cancer (36507). In addition, drinking black tea more than once a day, drinking strong black tea, or using more than 300 grams of tea leaves per month is associated with an approximately 2-fold increased risk of esophageal cancer when compared with drinking black tea up to once daily, drinking mild to moderate black tea, or using up to 300 grams of tea leaves per month (102756). Some evidence also shows caffeine, which is found in black tea, is associated with breast cancer in females. However, this is controversial since findings are conflicting (8043).

Pulmonary/Respiratory ...Orally, caffeine in black tea may cause tachypnea-induced respiratory alkalosis (11832,11838,13735). Some researchers think that stopping regular use of caffeine may cause withdrawal symptoms such as runny nose. General ...Topically, jasmine may cause allergic contact dermatitis, delayed-type hypersensitivity, and type 1 hypersensitivity (56955,57027,57032,98684).

Immunologic ...Topically, jasmine essential oil and jasmine flower can cause allergic contact dermatitis (56955,57027,57032). Among patients who are sensitive to fragrance materials, jasmine may cause delayed-type hypersensitivity. The reported prevalence of this type of reaction ranges from 0.4% to 1.6% (89684). One case of type 1 hypersensitivity to jasmine has also been reported. The patient experienced chest tightness, wheezing, itchy skin, and macular erythematous eruption within minutes of exposure to jasmine. The eruption subsided within 20 minutes of moving away from the plant. An immunoblotting assay revealed IgE reactivity (98684).

Dermatologic ...Orally, high doses of sea buckthorn may cause yellow staining of the skin. One case of generalized yellow skin has been reported for a patient who consumed sea buckthorn 100 grams daily for 6 months (74072). Topically, sea buckthorn oil has been associated with dryness, irritation, redness, and rash (74077,102845). Rash occurred in 4% of patients in one clinical study (74077). When used intravaginally, severe burning and itching have been reported rarely (106089).
