INSUFFICIENT RELIABLE EVIDENCE to RATE

• Diabetes. Although there has been interest in using oral alfalfa for diabetes, there is insufficient reliable information about the clinical effects of alfalfa for this purpose.
• Dyspepsia. Although there has been interest in using oral alfalfa for dyspepsia, there is insufficient reliable information about the clinical effects of alfalfa for this purpose.
• Hypercholesterolemia. It is unclear if oral alfalfa seeds are beneficial in patients with hypercholesterolemia. Details: A small, uncontrolled clinical study in patients with hypercholesterolemia shows that taking heat-prepared alfalfa seeds 40 grams three times daily for 8 weeks reduces total cholesterol and low-density lipoprotein (LDL) cholesterol levels when compared to baseline (5816). The validity of these findings is limited by the lack of a control group.
• Menopausal symptoms. Although there has been interest in using oral alfalfa for menopausal symptoms, there is insufficient reliable information about the clinical effects of alfalfa for this purpose. There is insufficient reliable information available about the effectiveness of alfalfa for its other uses.

POSSIBLY EFFECTIVE

• Colorectal cancer. Consumption of cruciferous vegetables, including broccoli, seems to reduce the risk of developing colorectal cancer. Details: Epidemiological research has found that the highest intakes of cruciferous vegetables, including broccoli, are associated with an 18% to 20% reduced risk of developing colorectal neoplasms when compared with the lowest intakes (14148,96187,96189).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Bladder cancer. It is unclear if consumption of broccoli reduces the risk of developing bladder cancer. Details: Epidemiologic research has found that eating about 1.75 cups daily of cruciferous vegetables, such as broccoli or cabbage, is associated with a 30% reduced risk of developing bladder cancer (14864).
• Breast cancer. It is unclear if consumption of broccoli reduces the risk of developing breast cancer. Details: Epidemiological research has found that consumption of broccoli is associated with a modestly reduced risk of developing breast cancer in premenopausal adults. However, broccoli consumption does not seem to be associated with breast cancer risk in postmenopausal adults (14146).
• Fibromyalgia. Broccoli has only been evaluated in combination with ascorbigen; its effect when used alone is unclear. Details: A small clinical study in females with fibromyalgia shows that taking one capsule containing ascorbigen 100 mg and broccoli powder 400 mg once daily for one month improves pain by 18% and reduces physical impairment by 20% when compared with baseline (19605). It is unclear if this effect is due to broccoli, ascorbigen, or the combination. The validity of these findings is limited by the lack of a comparator group.
• Gastric cancer. It is unclear if consumption of broccoli reduces the risk of developing gastric cancer. Details: A small epidemiological study has found that higher consumption of broccoli is associated with a reduced risk of developing gastric cancer when compared with lower consumption (14148).
• Hypercholesterolemia. Broccoli has only been evaluated in combination with cabbage; its effect when used alone is unclear. Details: A small clinical study in patients with mild to moderate hypercholesterolemia shows that consuming 160 grams of a beverage containing broccoli, cabbage, and fruit twice daily for 12 weeks reduces levels of low-density lipoprotein (LDL) cholesterol by 8.5% when compared with baseline. It is unclear if the improvement from baseline differed when compared with those consuming a similar beverage without broccoli and cabbage (19607). It is unclear if this effect is due to broccoli, cabbage, or the combination.
• Prostate cancer. It is unclear if consumption of broccoli reduces the risk of developing prostate cancer; the available research is conflicting. In patients with prostate cancer, broccoli has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Some epidemiological research has found that consumption of Brassica vegetables, such as broccoli, Brussels sprouts, cauliflower, and cabbage, is associated with a reduced risk of developing prostate cancer (14145). However, other epidemiological research has found no association (14147,15172). Clinical research in adults with prostate cancer shows that taking a whole food formulation containing broccoli powder 100 mg, turmeric powder 100 mg, pomegranate whole fruit powder 100 mg, and green tea extract 20 mg three times daily for 6 months prevents an increase in prostate specific antigen (PSA) levels. Mean PSA levels increased by 15% in the whole food formulation group, compared with 79% in the placebo group. Also, 46% of those taking the whole food formulation showed final PSA levels that were the same or lower than initial PSA levels, compared with 14% of those in the placebo group (89730). However, it is not yet known whether the whole food formulation reduces the risk of prostate cancer progression or recurrence. It is unclear if these effects are due to broccoli, other ingredients, or the combination. More evidence is needed to rate broccoli for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Dementia. It is unclear if consuming dewaxed brown rice improves cognitive function in adults with dementia. Details: A small clinical study in elderly nursing home residents with low cognitive function shows that eating dewaxed brown rice with 3 meals daily for 6 months improves scores on the Revised Hasegawa's Dementia Scale to a greater extent than eating white rice (103626).
• Diabetes. It is unclear if consuming brown rice as part of the diet is beneficial for improving glycemic control in adults with type 2 diabetes; the available research is conflicting. Details: Meta-analyses of clinical trials evaluating adults with type 2 diabetes shows that consuming a diet containing brown rice 50-450 grams daily for 4-104 weeks does not improve glycated hemoglobin (HbA1c) levels or fasting blood glucose levels when compared with a white rice diet (108661,108662). Clinical trials using non-glutinous brown rice for 24 hours or 16 weeks show similar results (96996,96998). However, not all research agrees. Preliminary clinical research in adults with type 2 diabetes shows that eating glutinous brown rice twice daily for 8 weeks modestly reduces HbA1c levels, compared with no change in those eating white rice (96995). Preliminary clinical research also shows that eating glutinous brown rice as the staple food over a 24-hour period helps with acute glycemic response when compared with white rice (96996). Other preliminary clinical research in females with type 2 diabetes shows that following a brown rice diet for 12 weeks reduces HbA1c and fasting blood glucose by 3% to 5% and reduces body weight and body fat by approximately 2% when compared with no intervention (18660). Clinical trials in adults with type 2 diabetes also report conflicting effects of brown rice on body weight, waist circumference, body fat, lipids, and blood pressure when compared with white rice (108661,108662). However, the limited studies showing positive benefits of brown rice report very minor effects with unclear clinical implications. The reasons for these discrepancies are unclear but may be related to the slight differences between the varieties of brown rice.
• Impaired glucose tolerance (Prediabetes). It is unclear if consuming brown rice as part of the diet is beneficial for improving glycemic control; the available research is conflicting. Details: A meta-analysis of clinical trials evaluating adults with prediabetes shows that consuming a brown rice-based diet for 6-12 weeks does not improve glycated hemoglobin (HbA1c) levels, fasting blood glucose levels, body weight, or waist circumference when compared with a white rice-based diet (108662). However, some preliminary clinical research in middle-aged females with prediabetes shows that replacing staple white rice in the diet with pre-germinated brown rice for 4 months modestly reduces fasting blood glucose and HbA1c levels when compared with baseline. There were also small improvements in body fat, body mass, systolic and diastolic blood pressure, and total cholesterol levels when compared with baseline (96997). The validity of these findings is limited by the lack of a comparator group.
• Metabolic syndrome. It is unclear if consuming brown rice reduces the prevalence of metabolic syndrome or improves metabolic parameters in patients with this condition. Details: Preliminary clinical research in adults with metabolic syndrome shows that eating brown rice ad libitum for 16 weeks does not improve metabolic factors or reduce the number of patients still meeting the diagnosis of metabolic syndrome when compared with eating white rice ad libitum (96998). However, other clinical research shows that eating 200 grams daily of cooked, pre-germinated brown rice for 3 months reduces the number of patients meeting the diagnosis of metabolic syndrome by 30%, compared with a reduction of 10% in those eating 200 grams daily of cooked white rice (103627). This suggests that any benefit might be related to regular consumption of brown rice. A small clinical study in overweight adults with metabolic syndrome shows that eating brown rice with 2 meals per day, 6 days per week for 3 months, reduces glycated hemoglobin (HbA1c) when compared with eating the same amount of white rice (103623). Other preliminary clinical research shows that eating sticky or glutinous brown rice for 8 weeks modestly improves body weight, some measures of insulin resistance, and levels of low-density lipoprotein (LDL) cholesterol when compared with baseline (96999). The validity of these findings is limited by the lack of a comparator group.
• Obesity. Although there is interest in using oral brown rice for weight loss, there is insufficient reliable information about the clinical effects of brown rice for this purpose.
• Psoriasis. Although there is interest in using oral brown rice for psoriasis, there is insufficient reliable information about the clinical effects of brown rice for this condition. More evidence is needed to rate brown rice for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Atopic dermatitis (eczema). It is unclear if oral hemp seed oil is beneficial in patients with eczema. Details: A small preliminary clinical trial in patients with eczema shows that taking hemp seed oil 30 mL daily for 8 weeks improves skin dryness, itchiness, and use of dermal medication when compared with baseline, but not when compared with an olive oil control (101125).
• Constipation. Oral hemp seed has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Clinical research in patients with functional constipation shows that taking a proprietary Chinese herbal medicine (Hemp Seed Pill) containing hemp seed, rhubarb root, peony root, apricot kernel, bitter orange fruit, and magnolia bark twice daily for 8 weeks results in a response rate to 43%, compared with 8% in those taking placebo. The response rate was defined as an increase of at least 1 complete spontaneous bowel movement per week. This combination product also improved the responder rate in the 2 weeks after treatment, as well as the overall use of rescue therapy, when compared with placebo (101127). Data from this study pooled with data from another larger study in adults with functional constipation shows that this same product increases the likelihood of complete response, defined as at least 3 complete spontaneous bowel movements per week or an increase over baseline of at least 1 weekly, by 1.43 times when compared with placebo. Complete spontaneous bowel movements per week also increased by 1 when compared with placebo (107319). It is unclear if these effects are due to hemp seed, other ingredients, or the combination.
• Dysmenorrhea. Although there is interest in using oral hemp for dysmenorrhea, there is insufficient reliable information about the clinical effects of oral hemp for this condition.
• Familial hypercholesterolemia. It is unclear if oral hemp seed oil is beneficial in children with this condition. Details: A small preliminary clinical trial shows that taking hemp seed oil (AlfaLife, Freia Farmaceutici Srl) 3 grams daily for 8 weeks does not improve levels of total or low-density lipoprotein (LDL) cholesterol when compared with no treatment in hyperlipidemic children aged 6-16 years who are also following a low-fat diet with high fruit and vegetable intake (101144).
• Joint pain. Although there is interest in using oral hemp for joint pain, there is insufficient reliable information about the clinical effects of oral hemp for this condition.
• Multiple sclerosis (MS). Oral hemp seed has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in patients with relapsing-remitting MS shows that taking a combination of hemp seed oil 5.4-6.3 grams and evening primrose oil three times daily for 6 months seems to improve disability scores and relapse rate when compared to baseline. However, the control group in this study ingested olive oil and also experienced notable improvements in disability and relapse rates (88183). This study is limited due to high dropout rate and because there was no statistical comparison between treatment and control groups.
• Obesity. It is unclear if an oral cannabidiol-rich hemp oil extract helps to improve weight loss. Details: A small clinical study in healthy overweight adults shows that taking a specific hemp oil extract (PlusCBD Extra Strength Hemp Extract, CV Sciences) as 60 mg, providing 15 mg cannabidiol, daily for 6 weeks, does not reduce weight or body mass index when compared with placebo (103805). The validity of this study is limited due to its exploratory nature and lack of control for voluntary dietary changes.
• Osteoarthritis. It is unclear if oral hemp seed is beneficial in patients with osteoarthritis. Details: A clinical study in 18 older adults with osteoarthritis undergoing in-hospital rehabilitation following knee or hip arthroplasty shows that consuming pasta enriched with hemp seed flour for 6 weeks improves pain scores by about 3 points, compared with only 1 point in those consuming a regular pasta control (107320).
• Psychological well-being. Although there is interest in using oral hemp for psychological well-being, there is insufficient reliable information about the clinical effects of oral hemp for this condition. More evidence is needed to rate hemp for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Bladder cancer. It is unclear if consuming kale in the diet can reduce the risk of bladder cancer. Details: Population research suggests that patients with bladder cancer consumed approximately 0.18 fewer cups of cruciferous vegetables, such as cabbage, Brussels sprout, cauliflower, broccoli, kale, or collards, on a weekly basis in the year prior to diagnosis when compared with people without bladder cancer. The effects of kale, specifically, are unclear (14864). Other population research suggests that eating at least five servings per week of cruciferous vegetables, such as cauliflower, Brussels sprout, cabbage, broccoli, and kale, is associated with a 51% reduction in the risk of developing bladder cancer in males when compared with eating up to one serving per week. However, higher intake of kale alone does not seem to be associated with a reduced risk of bladder cancer (26184).
• Breast cancer. It is unclear if consuming kale in the diet can reduce the risk of breast cancer. Details: Population research suggests that the highest dietary intake of cruciferous vegetables, including broccoli, Brussels sprout, cabbage, cauliflower, collards, and kale, is associated with a slight increase in the risk of breast cancer in premenopausal adults when compared with the lowest intake (26188). Cruciferous vegetables might increase urinary levels of 2-hydroxyestrone, and its ratio to 16-alpha-hydroxyestrone, which could reduce the risk of breast cancer. However, research in pre-menopausal adults shows that taking a dried kale and Brussels sprout supplement, 3.3 grams daily for 8 weeks, or increasing dietary intake of Brussels sprout or broccoli by one 40-gram serving daily, does not increase urinary levels of 2-hydroxyestrone, or its ratio to 16-alpha-hydroxyestrone, when compared with placebo (112086). In postmenopausal adults, the highest intake of cruciferous vegetables appears to be associated with a statistically insignificant reduced risk of breast cancer (26188).
• Inflammatory bowel disease (IBD). Although there is interest in oral kale for IBD, there is insufficient reliable information available about the use of kale for this purpose.
• Menopausal symptoms. Although there is interest in oral kale for reducing menopausal symptoms, there is insufficient reliable information available about the use of kale for this purpose. More evidence is needed to rate kale for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Cancer. Although there has been interest in using oral papain for cancer, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Dental caries. It is unclear if topical papain gel is beneficial for reducing procedure time or pain in children having caries removed by a dentist. Details: A small clinical study in children 3-9 years of age with dental caries shows that application of a specific papain gel (Brix3000, Brix Medical Science) 3000 U/mg during atraumatic restorative treatment (ART) by a dentist actually increases the time required to remove caries tissue, with no effect on pain, when compared with ART alone (107434).
• Diabetic foot ulcers. Although there has been interest in using topical papain for diabetic foot ulcers, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Exercise-induced muscle soreness. Oral papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in healthy males shows that taking a combination enzyme tablet containing papain 50 mg, trypsin 75 mg, bromelain 50 mg, amylase 10 mg, lipase 10 mg, lysosome 10 mg, and chymotrypsin 2 mg, four times in one day before downhill running, improves recovery of contractile muscle function and decreases muscle soreness when compared with placebo (67838). It is unclear if these findings are due to papain, other ingredients, or the combination.
• Herpes zoster (shingles). Although there has been interest in using oral papain for shingles, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Insect bite. It is unclear if topical papain is beneficial in individuals with fire ant stings. Details: A small clinical study in individuals with fire ant stings shows that applying gauze soaked with a specific papain-containing meat tenderizer (Adolph's meat tenderizer) to the sting for 20 minutes does not reduce pain or itching when compared with placebo (67845).
• Intestinal parasite infection. Although there has been interest in using oral papain for intestinal parasite infections, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Jellyfish stings. It is unclear if topical papain is beneficial in individuals with jellyfish stings. Details: A small clinical study in individuals with jellyfish stings shows that submerging affected skin into a solution with a papain-containing meat tenderizer (Adolph's meat tenderizer), starting two minutes after a jellyfish sting and continuing for at least 20 minutes, is less effective than hot water alone in decreasing pain caused by the jellyfish sting (67835).
• Pressure ulcers. Although there has been interest in using topical papain for pressure ulcers, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Radiation dermatitis. Oral papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: One small clinical study in patients receiving pelvic radiotherapy shows that taking three capsules of a specific combination product (Wobe-Mugos E, Mucos Pharma) containing papain 100 mg, trypsin 40 mg, and chymotrypsin 40 mg per capsule, four times daily, beginning 3 days prior to radiotherapy and continuing until the last day of therapy, does not attenuate skin deterioration when compared with placebo (67834). However, another small, open-label clinical study in patients with uterine carcinoma shows that taking three capsules of the same combination product four times daily, beginning 7 days before radiotherapy and continuing for 9 weeks, reduces skin reactions when compared with control (67831). It is unclear if these findings are due to papain, other ingredients, or the combination.
• Radiation-induced cystitis. Oral papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in patients with uterine carcinoma receiving radiotherapy shows that taking three capsules of a specific combination product (Wobe-Mugos E, Mucos Pharma) containing papain 100 mg, trypsin 40 mg, and chymotrypsin 40 mg per capsule, four times daily, beginning 7 days before radiotherapy and continuing for 9 weeks, reduces genitourinary symptoms when compared with control (67831).
• Radiation-induced nausea and vomiting. Oral papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in patients receiving pelvic radiotherapy shows that taking three capsules of a specific combination product (Wobe-Mugos E, Mucos Pharma), containing papain 100 mg, trypsin 40 mg, and chymotrypsin 40 mg per capsule, four times daily, beginning 3 days prior to radiotherapy and continuing until the last day of therapy, does not reduce radiation-related nausea or vomiting when compared with placebo (67834).
• Tonsillopharyngitis. Oral papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: Two small clinical studies in patients with pharyngitis and/or tonsillitis shows that use of throat lozenges containing papain 2 mg, lysozyme 5 mg, and bacitracin 200 IU daily for 4 days reduces pain and inflammation of the throat and lymph nodes when compared with placebo or disinfectant alone (964,968). It is unclear if these findings are due to papain, other ingredients, or the combination.
• Traumatic oral ulcers. Although there has been interest in using topical papain for traumatic oral ulcers, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Venous leg ulcers. Although there has been interest in using topical papain for venous leg ulcers, there is insufficient reliable information about the clinical effects of papain for this purpose.
• Wound healing. Topical papain has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in patients with purulent wounds shows that applying papain 1% solution plus dimethylsulfoxide (DMSO) followed by phonophoresis, a process by which ultrasound is used to increase topical drug delivery, improves wound healing when compared to baseline (67843). The validity of this finding is limited by the lack of a comparator group. Also, it is unclear if these findings are due to papain, DMSO, or the combination. More evidence is needed to rate papain for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Diabetes. Although there has been interest in using oral pea protein for diabetes, there is insufficient reliable information about the clinical effects of pea protein for this purpose.
• Exercise-induced muscle damage. It is unclear if oral pea protein is beneficial for exercise-induced muscle damage. Details: A small clinical study in healthy, untrained males shows that taking a specific type of pea protein (Nutralys S85 Plus, Roquette) 0.3 grams/kg 3 times daily for 5 days after a 90-minute whole-body high-intensity eccentric exercise bout does not reduce surrogate markers of exercise-induced muscle damage, including creatine kinase, lactate dehydrogenase, and myoglobin, when compared with whey protein or water (104758). However, the study may have been inadequately powered to detect a difference between groups and is further limited by double-blinding restricted only to the protein supplements. The water control did not match the taste, texture, or calorie content of the protein supplements. Another small clinical trial in healthy, physically active older adults shows that taking pea protein 25 grams daily for 10 days prior to and 3 days after a single 20-30 km endurance walk does not improve markers of muscle damage when compared with placebo and may be less effective when compared with whey protein (111729).

• Hypertension. It is unclear if oral pea protein hydrolysate is beneficial for hypertension. Details: A small clinical study in adults with elevated blood pressure shows that taking a pea protein hydrolysate 3 grams daily for 3 weeks reduces systolic blood pressure by 6 mmHg when compared with placebo. Taking the same product at a dose of 1.5 grams daily does not reduce blood pressure (94973).
• Irritable bowel syndrome (IBS). Oral pea protein has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small open-label study in patients with diarrhea-predominant IBS shows that taking two tablets of a specific product (Gelsectan, Noventure) containing pea protein, grape seed extract tannins, xyloglucan, and xylo-oligosaccharides (doses unknown) before breakfast and dinner daily for 6 months improves IBS-related symptoms, including diarrhea, pain, bloating, defecatory frequency, stool consistency, and defecatory urgency when compared to baseline (108881). The validity of this study is limited by a lack of placebo control group.
• Muscle strength. Clinical research on the impact of oral pea protein on muscle strength is mixed. Its effects might be similar to whey protein. Pea protein may be more effective in individuals with lower baseline muscle strength. Details: A clinical trial in physically active, but non-resistance trained, young male adults participating in a 12-week upper body resistance training program shows that taking a specific type of pea protein isolate (Nutralys, Roquette) 25 grams twice daily over the training program does not augment training-induced increases in muscle thickness or strength when compared with whey protein or isocaloric placebo. However, a post hoc analysis of participants with the lowest muscle strength at baseline shows that pea protein supplementation increases bicep muscle thickness more than placebo and to a similar degree as whey protein (95426). Additionally, a very small clinical study in trained adults shows that taking pea protein 24 grams twice daily during an 8-week high-intensity functional training program improves muscle strength to a similar degree as whey protein, while neither supplement appears to impact muscle thickness or body mass (102013). The small study size and lack of an isocaloric, protein-free control limits conclusions that can be drawn from this study.
• Obesity. Although there has been interest in using oral pea protein for obesity, there is insufficient reliable information about the clinical effects of pea protein for this purpose.
• Postoperative recovery. Oral pea protein has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in adults undergoing gastrointestinal resection due to cancer shows that consuming a beverage (Providextra, Fresenius Kabi) containing pea protein hydrolysate 11% and carbohydrates 89% the night before (400 mL) and the morning of surgery (200 mL) reduces postoperative hospital stay by almost 2-fold when compared with conventional preoperative fasting (94933). It is unclear if these findings are due to pea protein, carbohydrates, or the combination. More evidence is needed to rate pea protein for these uses.

Details: Preliminary clinical research in healthy adults shows that consuming spinach 200 grams with lunch does not improve attention or memory after 150 minutes when compared with a rice milk control (96852).
• Muscle strength. It is unclear if oral spinach is beneficial for muscle strength. Details: A small, preliminary clinical study in healthy adults over 50 years of age shows that taking an aqueous spinach extract, 1 gram orally twice daily, in conjunction with a moderate-intensity exercise program, 3 sessions per week of 1 hour each for 12 weeks, improves several measures of muscle strength and quality in both sexes, and increases lower leg muscle mass in males, when compared with placebo (109546).
• Nonalcoholic fatty liver disease (NAFLD). It is unclear if oral spinach can reduce the risk of developing NAFLD. Details: A large observational study has found that raw, but not boiled, spinach consumption is inversely associated with the risk of developing NAFLD (106566).
• Obesity. It is unclear if consuming spinach is beneficial for weight loss. Details: Preliminary clinical research in overweight or obese middle-aged and older females shows that taking a specific spinach product (Appethyl, Greenleaf Medical AB) 5 grams daily before breakfast for 12 weeks increases weight loss by an additional 1.5 kg when compared with placebo. However, fat loss, waist circumference, and blood lipids are not improved when compared with placebo (96856,96857).
• Polycystic ovary syndrome (PCOS). It is unclear if oral spinach-derived thylakoids are beneficial for PCOS. Details: A small clinical study in premenopausal adults with PCOS and obesity shows that adding 5 grams orally daily of spinach-derived thylakoids to a low-calorie diet for 12 weeks reduces weight and body mass index (BMI) by 8%, compared with 4% with placebo and the low-calorie diet. Insulin resistance is also improved among those receiving spinach extract. These changes are unlikely to be clinically significant (106568). More evidence is needed to rate spinach for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Diabetes. It is unclear if oral stevia is beneficial in patients with diabetes; the available research is conflicting. Details: A small single-dose study shows that taking stevia leaf extract 1000 mg, standardized to contain 91% stevioside, along with a meal reduces postprandial glucose levels by 18% in adults with type 2 diabetes when compared with a starch control (11812). However, clinical research in patients with type 1 or type 2 diabetes shows that taking stevioside 250 mg three times daily for 3 months does not affect blood glucose levels or glycated hemoglobin (HbA1C) when compared with baseline (16705). Also, a small clinical study in patients with diabetes shows that drinking 200 mL of black tea, sweetened with stevia extract 2%, three times daily for 8 weeks does not affect glycemic indices when compared with drinking tea sweetened with sucralose (103695).
• Hypertension. It is unclear if oral stevia is beneficial in patients with hypertension; the available research is conflicting. Details: Some clinical research in patients with mild to moderate hypertension shows that taking stevioside, a constituent of stevia, 750-1500 mg in three divided doses daily reduces systolic blood pressure by 10-14 mmHg and diastolic blood pressure by 6-14 mmHg within one week to three months of treatment initiation and continuing for up to 2 years (11809,11810). However, other clinical research in patients with mild hypertension shows that taking stevioside in doses up to 15 mg/kg daily does not significantly reduce blood pressure (16706).
• Obesity. It is unclear if oral stevia is beneficial in patients with obesity; the available research is conflicting. Details: Although a small clinical study in healthy adults shows that drinking water with stevia (Truvia) 245 ppm as a single dose reduces appetite and energy intake when compared with drinking water alone (103697), other small, single-dose studies have shown no effect of stevia 15.3 mg or 1 gram as a single dose on satiety, energy intake, or fullness when compared with either water alone or a non-nutritive sweetener (103696,110124). A small clinical study in healthy adults shows that taking an aqueous stevia leaf extract (SweetLeaf) twice daily for 12 weeks does not affect body weight, compared with an increase in weight in the control group. This suggests that stevia extract may have attenuated weight gain in this study (106458). More evidence is needed to rate stevia for these uses.

POSSIBLY EFFECTIVE

• Constipation. Taking xanthan gum orally as a bulk-forming laxative seems to decrease constipation (4917,4918).
• Swallowing dysfunction. Using a specific xanthan gum product (Resource ThickenUp Clear, Nestle Health Science) as a thickener in liquid feeds improves swallowing and reduces the risk of aspiration in people with swallowing dysfunction similarly to a modified starch thickener (Resource ThickenUp, Nestle Health Science). Xanthan gum also does not appear to leave the pharyngeal residues seen with the modified starch product (100912,100913).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Diabetes. Taking xanthan gum 12 grams orally daily in muffins seems to lower blood glucose in people with diabetes (4916).
• Dry eye. Using eye drops containing xanthan gum and chondroitin sulfate (PRO-148 or Xiel ofteno, Sophia Laboratories) four times daily for 60 days improves tear film break-up times, ocular surface disease index, and symptoms of dry eye similarly to polyethylene glycol/propylene glycol eye drops (Systane, Alcon Laboratories Inc.) (89591,95794). It is not clear if this effect is due to xanthan gum, chondroitin sulfate, or the combination.
• Sjogren syndrome. Applying xanthan gum topically in the mouth seems to help as a saliva substitute in people with Sjogren syndrome (4915).

However, there was no effect of hemp seed protein on blood pressure in normotensive animals (101136). Furthermore, hempseed oil consumption does not seem to reduce blood pressure in humans (101144).

Dermatologic ...Dermatitis associated with alfalfa use has been reported. In a 1954 publication, dermatitis was noted in a 61-year-old female consuming 4-6 cups of tea made with two tablespoonfuls of alfalfa seeds for approximately two months prior to onset. Examination revealed diffuse, confluent edema and erythema on the face, eyelids, ears, hands, forearms, and distal humeral regions. The dermatitis improved with treatment; re-exposure to alfalfa resulted in a similar reaction (30609).

Endocrine ...Alfalfa contains constituents, including coumestrol, with reported estrogenic activity (30586,30592,4753). Effects in humans are not known.
One case report documents hypokalemia in a female who had been drinking a "cleansing tea" containing alfalfa, licorice, and stinging nettle. The potassium level returned to normal after discontinuing the tea and initiating potassium supplementation. The specific cause of the hypokalemia is not clear. Notably, both stinging nettle and licorice have been associated with hypokalemia and may have been responsible for this effect (30562).

Gastrointestinal ...Orally, flatulence and bulkier feces were reported during the first week of a case series of three subjects ingesting alfalfa (30598). In a case series of 15 patients ingesting alfalfa, increased fecal volume and increased stool frequency was reported. Additional adverse effects included abdominal discomfort in two patients, diarrhea in two patients, loose stools in six patients, and intestinal gas in 13 patients (5816).

Hematologic ...Pancytopenia and splenomegaly were reported in a 59-year-old male who had been taking 80-160 grams of ground alfalfa seeds for up to six weeks at a time, for a five month period. Hematologic values and spleen size returned to normal when alfalfa was discontinued (381).

Other ...Alfalfa products, including sprouts, seeds, and tablets, have been found to be contaminated with Escherichia coli, Salmonella, and Listeria monocytogenes, which have caused documented infections (5600,30566,30568,30572,30569,30564,30604,30610,30563,30607) (30566,30564,30604,30610,30563,30607,30576).
Orally, alfalfa has been associated with the development of a lupus-like syndrome in animals and humans (30594,14828,14830,30602), as well as with possible exacerbations of lupus in patients with known systemic lupus erythematosus (SLE). These reactions may be associated with the amino acid L-canavanine (30594), which appears to be present in alfalfa seeds and sprouts, but not leaves, and therefore should not be present in alfalfa tablets manufactured from the leaves (30601). However, case reports have included individuals ingesting tablets. A lupus-like syndrome was described in four patients taking 12-24 alfalfa tablets per day. Symptoms included arthralgias, myalgias, and rash; positive antinuclear antibodies (ANA) arose anywhere from three weeks to seven months after initiating alfalfa therapy. Upon discontinuation of alfalfa tablets, all four patients became asymptomatic. In two patients, ANA levels normalized (14828). Two additional reports have documented possible exacerbation or induction of SLE associated with alfalfa use. One case involved a female with a 26-year history of SLE, who had been taking 15 tablets of alfalfa daily for nine months prior to an exacerbation. Because of the delay in onset of the exacerbation from the initiation of alfalfa therapy, causation cannot be clearly established (30575). In a different report, SLE and arthritis were found in multiple family members who had been taking a combination of vitamin E and alfalfa tablets for seven years (30602). It is not known what other environmental or genetic factors may have affected these individuals, and the association with alfalfa is unclear.

This was thought to be related to the consumption of 800 mL of broccoli juice daily over a 4-week period. Values returned to normal 15 days after cessation of juice consumption (96191).

Cardiovascular ...Acquired long QT syndrome, torsades de pointes, and syncope have been reported in a 56-year-old woman following the intake of supplements containing hemp oil. The hemp supplements provided cannabidiol (CBD), and possibly cannabigerol (CBG). Although the exact dose is unknown, up to six times the recommended dose had been used for approximately 6 weeks, in combination with a supplement containing berberine. While hospitalized, intravenous magnesium and saline were used to stabilize heart rhythm. It is unknown whether this adverse effect was related to the hemp oil, berberine, or their interaction (110104).

Hepatic ...Orally, there is a case report of elevated liver enzymes and hepatitis in a two-year-old boy given hemp extract 2. 5 mL, providing 125 mg phytocannabinoid, five to eight times daily for infantile spasms and refractory seizures. The total dose of phytocannabinoids was approximately 60-100 mg/kg daily (110287).

Immunologic ...Orally, there are case reports of allergy to hemp seed, although this is uncommon (101140,101154). A 44-year-old male developed hives during a meal of hemp seed-crusted seafoods. Later, he developed facial swelling, shortness of breath, and problems speaking. Evaluation revealed allergy to a specific protein in hemp seed. He did not react to smoked cannabis (101140). In other cases, anaphylaxis, facial swelling, and worsening asthma have been reported in association with a first exposure to hemp seed, although some had smoked cannabis previously (101154).
Topically, a case of patch-test confirmed allergic contact dermatitis to hemp seed oil has been reported in a 22-year-old woman. The initial rash started at the application point on her back and spread to her arms, hands, and neck (110288).
Airborne exposure to hemp pollen is a relatively common cause of allergic respiratory symptoms in some locations (101155).

Neurologic/CNS ...Orally, cases of acute cannabinoid toxicity with neurological symptoms in children and adults have been associated with intake of hemp seed oil. There is a case report of decreased alertness, stupor, bloodshot eyes, and fixed gaze in a 2-year-old male probably related to the intake of one teaspoon hemp seed oil (CANAH) containing 0.06% delta-9-tetrahydrocannabinol (THC) twice daily for 3 weeks. After stopping the oil, irritability was reported over the next few days (101153).

Dermatologic ...Topically, papain can cause itching (966). Urticarial reactions and itching have been reported in people occupationally exposed to papain, with papain confirmed as the causative agent by skin prick tests or radioallergosorbent tests (RAST) (95533,95534). In a randomized controlled trial assessing the effects of papain, trypsin, and chymotrypsin on adverse effects from radiotherapy, moderate to severe epitheliolysis was more frequent in the enzyme-treated group than the placebo group (67834). It is unclear if this adverse effect is due to papain, other enzymes, or the combination.

Gastrointestinal ...Orally, papain has been associated with diarrhea. In a randomized controlled trial assessing the effects of papain, trypsin, and chymotrypsin on adverse effects from radiotherapy, moderate to severe diarrhea was more frequent in the enzyme-treated group than the placebo group (67834). However, it is unclear if this adverse effect is due to papain, other enzymes, or the combination. Papain has also been associated with gastric ulcers and esophageal perforation in case reports of phytobezoars treated with papain (67848). In general, large amounts of papain can cause esophageal perforation (6). Ingestion of papaya latex (raw papain) can cause severe gastritis.

Genitourinary ...Orally, papain has been associated with hypernatremia in case reports of phytobezoars treated with papain (67848).

Immunologic ...Orally, papain may cause allergic reactions, including itchy watery eyes, runny nose, sneezing, abdominal cramps, sweating, and diarrhea, in individuals sensitive to papain (6,967). Occupational exposure to airborne papain dust may also cause respiratory allergic reactions (95532,95533,95534,95535,95536).

Pulmonary/Respiratory ...Occupational exposure to airborne papain dust may cause respiratory allergic reactions. Symptoms include rhinitis, sneezing, conjunctivitis, dyspnea, wheezing, cough, and asthma. In most cases, papain is confirmed as the causative agent by skin prick tests, radioallergosorbent tests (RAST), or detection of papain-specific immunoglobulin E (IgE) and IgG (95532,95533,95534,95535,95536).

Hematologic ...Orally, spinach ingestion by infants under 4 months of age can cause methemoglobinemia, due to its high nitrate content (75802,75858,75860,75861,75862).

Immunologic ...Orally, topically, and via inhalation, spinach has been reported to cause allergic reactions in sensitive individuals (75870,96859).

Pulmonary/Respiratory ...Lung inflammation associated with allergic alveolitis has been reported after inhalation of spinach powder (75871). The powder has also been reported to induce occupational asthma in a spinach factory worker (75833).

Musculoskeletal ...Orally, stevia and steviol glycosides may cause myalgia, but this generally resolves after the first week of use (11809,11810).

Neurologic/CNS ...Orally, stevia and steviol glycosides may cause headache, dizziness, and numbness (11809,11810).

Immunologic ...An itchy, burning dermatitis was reported in a 9-year old girl after application of a sunscreen product containing xanthan gum. Patch testing with the separate ingredients of the sunscreen identified xanthan gum as the reacting agent, and demonstrated a dose-dependent effect with 1% and 10% solutions (100914).

Pulmonary/Respiratory ...Occupational exposure in workers handling xanthan gum powder can cause flu-like symptoms and nose and throat irritation without acute or chronic loss of pulmonary function (4913).
