POSSIBLY INEFFECTIVE

• Athletic performance. Taking cordyceps orally does not seem to improve athletic performance in adults. Details: One small clinical study in endurance-trained cyclists shows that taking a specific mycelial fermentation product of cordyceps (CordyMax Cs-4) 3.15 grams daily for 5 weeks does not improve endurance time trials or aerobic capacity when compared with placebo (12095). Another small study in healthy elderly adults shows that taking the same cordyceps preparation (CordyMax Cs-4) 3 grams daily for 12 weeks does not improve exercise performance when compared with placebo (46120). Research also shows that taking combination products containing cordyceps and other ingredients does not improve athletic performance when compared with control. These products have combined cordyceps (CordyMax Cs-4) 1000 mg with rhodiola 300 mg (46091) or cordyceps (CordyMax Cs-4) 2000 mg with roseroot 600 mg (Optygen, First Endurance) (15573); liquid and powdered cordyceps formulations with reishi mushroom, shiitake mushroom, bamboo, and honey (Fohow oral liquid and Fohow lingzhi capsules, FOHOW Technology Investment Group Co. Ltd.) (98686); and cordyceps (form unknown) 1.2 grams with a 930 mg mixture of extracts of ashwagandha root, green tea leaf, rhodiola root, and astragalus root (Shroom Tech Sport, Onnit Labs) (105079).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Arrhythmia. Although there is interest in using oral cordyceps for arrhythmia, there is insufficient reliable information about the clinical effects of cordyceps for this condition.
• Asthma. Oral cordyceps has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in children with asthma shows that a 619 mg capsule containing equal weights of the dried aqueous extracts of cordyceps, astragalus, skullcap, Radix stemonae, and Bulbus fritillariae daily for 6 months does not reduce the need for medication or improve symptoms when compared with placebo (32935).
• Chronic kidney disease (CKD). Small clinical studies suggest that oral cordyceps may modestly improve biomarkers in some patients with CKD. Details: A meta-analysis of clinical research in patients in China with CKD shows that taking cordyceps 0.6-2 grams orally three times daily, along with standard treatment, seems to decrease serum creatinine levels by 0.6 mg/dL and increase creatinine clearance by 9.2 mL/min when compared with standard treatment alone (92829). Another meta-analysis of clinical studies in patients in China with diabetic kidney disease shows that taking cordyceps 0.5-2 grams orally three times daily along with angiotensin-converting enzyme inhibitors (ACEI) or angiotensin receptor blockers (ARB) decreases blood urea nitrogen by 2 mg/dL and decreases serum creatinine levels by 0.1 mg/dL when compared with ACEI/ARB treatment alone (92830). Most individual studies in these analyses used Bailing (Cs-C-Q80) or Jinshuibao (Cs-4), fermented cordyceps mycelial products (92829,92830). Both of these analyses are limited by the low methodological quality and short duration (usually 1-6 months) of most of the included studies.
• Chronic obstructive pulmonary disease (COPD). Preliminary clinical studies suggest that oral cordyceps may modestly improve symptoms of COPD. Details: A meta-analysis of preliminary clinical research in patients with moderate or severe COPD shows that taking cordyceps alone or as part of a formulation containing other products for up to 12 months improves lung function and reduces the incidence of an acute exacerbation by a small amount. Also, the distance walked in 6 minutes is increased by about 45 meters, which was considered clinically meaningful. Most individual studies in this analysis used Bailing (Cs-C-Q80) or Jinshuibao (Cs-4), fermented cordyceps mycelial products, in combination with conventional treatments with conventional treatments alone (109705). This analysis is limited by the low methodological quality of the included studies.
• Contrast induced nephropathy. Small clinical studies suggest that oral cordyceps may modestly reduce contrast induced nephropathy. Details: A preliminary clinical study in patients with diabetic nephropathy shows that taking a specific cordyceps mycelial product (Corbrin; CS-C-Q80) 2-3 grams orally three times daily for 3 days before and after angiography reduces the risk of contrast induced nephropathy by about 48% to 66% when compared to standard treatment (92827). However, other preliminary clinical research in patients with stable angina pectoris shows that taking the same cordyceps product 3 grams orally three times daily for 3 days before and after angioplasty does not decrease the prevalence of contrast induced nephropathy when compared to standard therapy (92826). Reasons for these discrepancies are not clear but may relate to the small size and heterogenous populations in these studies.
• Diabetic nephropathy. Early research suggests that oral cordyceps may modestly improve kidney function in patients with diabetic kidney disease. Details: A meta-analysis of low to moderate quality clinical research in patients with diabetic kidney disease shows that taking cordyceps as adjunctive therapy to angiotensin-converting enzyme inhibitors or angiotensin receptor blockers for 2-24 weeks modestly improves kidney function when compared to these medications alone. Kidney function was based on markers such as blood creatinine and urea nitrogen, as well as urinary albumin and total protein. There were also modest improvements in systolic and diastolic blood pressure, triglycerides, and low-density lipoprotein (LDL) cholesterol. There was no effect on glycemic indices. The effect of cordyceps on disease progression or cardiovascular events is unclear (111903).
• Cough. Although there is interest in using oral cordyceps for cough, there is insufficient reliable information about the clinical effects of cordyceps for this purpose.
• Fatigue. Although there is interest in using oral cordyceps for fatigue, there is insufficient reliable information about the clinical effects of cordyceps for this purpose.
• Hepatitis B. Although there is interest in using oral cordyceps for hepatitis B, there is insufficient reliable information about the clinical effects of cordyceps for this condition.
• Hypercholesterolemia. Although there is interest in using oral cordyceps for hypercholesterolemia, there is insufficient reliable information about the clinical effects of cordyceps for this condition.
• Kidney failure. Small clinical studies suggest that oral cordyceps may improve inflammatory markers in patients on hemodialysis. Details: A meta-analysis of small clinical trials in patients in China on hemodialysis suggests that adding cordyceps to standard treatment does not seem to improve serum creatinine levels but may improve certain markers of inflammation, such as C-reactive protein, when compared with placebo. Most individual studies in this analysis used Bailing (Cs-C-Q80) or Jinshuibao (Cs-4), fermented cordyceps mycelial products (105076).
• Kidney transplant. Small clinical studies suggest that oral cordyceps is no better than standard treatment in patients with kidney transplant. Details: A meta-analysis of 4 heterogeneous clinical trials evaluating patients in China immediately post-kidney transplant or patients with complications post-kidney transplant suggests that adding cordyceps to standard therapy with cyclosporine and steroids is no better than adding azathioprine 50-150 mg for improving overall survival or graft survival or reducing rejection risk. However, there was a non-significant trend to reduced risk of graft rejection in the cordyceps group (95905). A higher quality systematic review, which determined the available studies were too heterogeneous to pool together in a meta-analysis, suggests that adding cordyceps 3-6 grams to standard therapy daily is no better than azathioprine or cyclosporine for improving organ rejection, kidney function, or survival in patients after kidney transplant (92828). All individual studies in these analyses used Bailing (Cs-C-Q80) fermented cordyceps mycelial product (95905,92828).
• Sexual dysfunction. It is unclear if oral cordyceps is beneficial in patients with sexual dysfunction. Details: Preliminary clinical research shows that taking a specific mycelial fermentation product of cordyceps (CordyMax Cs-4) approximately 3 grams daily for 40 days can improve symptoms of hyposexuality in 66% of patients, compared with 32% of patients taking natural cordyceps and 24% of patients taking placebo (46145).
• Tinnitus. Although there is interest in using oral cordyceps for tinnitus, there is insufficient reliable information about the clinical effects of cordyceps for this condition. More evidence is needed to rate cordyceps for these uses.

POSSIBLY EFFECTIVE

• Acne. Application of a topical pyruvic acid skin peel may improve acne severity in some patients. Details: A large clinical trial in females with mild to moderate acne shows that applying a pyruvic acid 50% skin peel every 2 weeks for 10 weeks reduces acne severity when compared to baseline. Pyruvic acid showed similar improvements in acne severity while also improving skin oiliness when compared with an azelaic acid skin peel (108892).
• Obesity. Oral pyruvate may slightly reduce body weight in overweight and obese patients when combined with diet and/or exercise. Details: Several small clinical studies in patients with obesity or hyperlipidemia show that taking pyruvate 6-44 grams daily for up to 6 weeks in place of a portion of dietary carbohydrates and in combination with a low-calorie diet seems to help increase weight loss and decrease body fat (2472,2474,13658). However, a small clinical trial in overweight females shows that taking pyruvate 5 grams twice daily for 30 days while participating in a supervised exercise program does not reduce body weight when compared with placebo (13660). A meta-analysis of these studies shows that taking pyruvate 6-44 grams daily for up to 6 weeks reduces body weight by an average of 1.6 pounds when combined with varying diets and levels of exercise (92511).

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Alcohol-related liver disease. It is unclear if intravenous sodium pyruvate is beneficial in patients with alcohol-related liver disease. Oral pyruvate has not been evaluated for this condition. Details: A very small clinical study in patients with alcohol-related liver disease shows that infusing sodium pyruvate 54-86.4 grams daily for 10 days and then 50-54 grams daily for 15 days improves markers of liver dysfunction, including aspartate aminotransferase, alanine aminotransferase, and bilirubin levels, when compared to baseline (70256). The validity of these findings is limited by the lack of a comparator group.
• Athletic performance. While research is mixed, most clinical evidence suggests that oral pyruvate does not improve athletic performance when taken alone. Details: A very small clinical study in well-trained cyclists shows that taking pyruvate 7 grams daily for one week does not improve time trial performance or measures of exercise capacity when compared with placebo (70255). Another small clinical study in American football players shows that taking pyruvate 0.22 grams/kg daily for 5 weeks does not improve jump height or peak cycling power when compared with placebo (13661). However, a small crossover trial in male soccer athletes shows that taking sodium pyruvate 0.1 gram/kg daily for one week improves repeated sprint exercise performance when compared with placebo (108893). Pyruvate has also been evaluated in combination with other ingredients. One small clinical study in well-trained cyclists shows that taking a combination of pyruvate and creatine 7 grams daily for one week does not improve endurance capacity or sprint performance when compared with placebo (13772). However, two very small clinical studies in untrained males show that consuming high-dose dihydroxyacetone plus pyruvate 25 grams daily for one week increases arm and leg exercise endurance when compared with placebo (807,808). It is unclear if these findings are due to pyruvate, dihydroxyacetone, or the combination.
• Atrophic acne scars. It is unclear if application of a topical pyruvic acid skin peel improves atrophic acne scars. Details: A small clinical study in adults with atrophic acne scars treated with microneedling shows that applying a pyruvic acid 25% peel once modestly improves acne scarring and patient- and physician-reported scores at 21 weeks when compared to baseline but not when compared with a trichloroacetic acid 15% peel (112174). The interpretation of these findings is limited by the short duration of follow-up, as certain scars may need an extended time for improvement.
• Atrophic acne scars. It is unclear if application of a topical pyruvic acid skin peel improves atrophic acne scars. Details: A small clinical study in adults with atrophic acne scars treated with microneedling shows that applying a pyruvic acid 25% peel once modestly improves acne scarring and patient- and physician-reported scores at 21 weeks when compared to baseline but not when compared with a trichloroacetic acid 15% peel (112174). The interpretation of these findings is limited by the short duration of follow-up, as certain scars may need an extended time for improvement.
• Cataracts. Although there has been interest in using oral pyruvate for cataracts, there is insufficient reliable information about the clinical effects of pyruvate for this purpose.
• Chronic obstructive pulmonary disease (COPD). It is unclear if inhaled sodium pyruvate is beneficial in patients with COPD. Details: A small clinical study in patients with moderate to severe COPD shows that inhaling nebulized sodium pyruvate 275 mcg three times daily for 6 weeks may improve lung function, as measured by forced expiratory volume in one second (FEV1), by up to 11% when compared with placebo (70278).
• Congestive heart failure (CHF). Small clinical studies suggest that intravenous pyruvate may improve measures of cardiac function in patients with CHF. Oral pyruvate has not been evaluated for this purpose. Details: Two very small, open-label clinical studies in patients with CHF show that infusing pyruvate 150-300 mmol/L into the left main coronary artery (for a total dose of 27-41.6 mmol pyruvate) may increase left ventricular ejection fraction (LVEF), increase cardiac index by 23%, increase stroke-volume index by 38%, and decrease pulmonary capillary wedge pressure by 36% when compared to baseline (70253,70262). The validity of these findings is limited by the lack of a comparator group.
• Glaucoma. Oral pyruvate has only been evaluated in combination with other ingredients; its effect when used alone is unclear. Details: A small clinical study in adults with open-angle glaucoma and optic neuropathy shows that taking pyruvate and niacinamide, titrated to 3 grams of each, daily for 3 weeks increases the odds of pointwise improvement in visual function by 3.2-fold when compared with placebo. However, global visual field index and retinal nerve fiber thickness were not improved (107708).
• Hyperlipidemia. It is unclear if oral pyruvate is beneficial in patients with hyperlipidemia. Details: A small clinical study in patients with hyperlipidemia shows that supplementing a high-cholesterol, high-fat diet with pyruvate 36-53 grams daily in three divided doses for 6 weeks decreases total cholesterol by 4% and low-density lipoprotein (LDL) cholesterol by 5% when compared with placebo (70259).
• Ichthyosis. It is unclear if topical pyruvate is beneficial in patients with ichthyosis. Details: A small clinical study in patients with different types of ichthyosis shows that applying a Eucerin skin cream containing pyruvate 5% daily for at least 2 weeks increases shedding of scaly skin when compared with Eucerin alone (70285).
• Wrinkled skin. It is unclear if a topical pyruvic acid skin peel is beneficial in patients with wrinkled skin. Details: A small clinical study in patients with photodamaged facial skin shows that applying a pyruvic acid 50% skin peel once weekly for 4 weeks smooths skin, decreases fine wrinkles, and decreases hyperpigmentation when compared to baseline (12020). The validity of these findings is limited by a lack of control group. More evidence is needed to rate pyruvate for these uses.

INSUFFICIENT RELIABLE EVIDENCE to RATE

• Antidepressant-induced sexual dysfunction. It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial for patients with antidepressant-induced sexual dysfunction. Details: Small clinical trials suggest that taking yohimbine, a constituent of yohimbe, improves sexual dysfunction associated with selective serotonin reuptake inhibitors (SSRIs) (3305,3306,3307,3311,3313,3970,3971,3972,3973). It is unclear if yohimbe bark is beneficial.
• Anxiety. There is limited evidence on the oral use of yohimbine, a constituent of yohimbe, for the management of various phobias. Details: A small clinical study in healthy adults with a fear of flying shows that taking yohimbine hydrochloride 10 mg as an adjunct to virtual reality exposure therapy (VRET) does not improve anxiety measures when compared with VRET alone (86819). However, another small clinical study in healthy adults with claustrophobia shows that taking yohimbine hydrochloride 10.8 mg prior to exposure-based treatment reduces peak fear after one week when compared with exposure-based treatment alone (86794). The effect of yohimbe bark has not been investigated in clinical research.
• Athletic performance. It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial for athletic performance. Details: A small clinical study in professional soccer players shows that taking yohimbine, a constituent of yohimbe, 20 mg daily for 3 weeks does not improve exercise performance or build muscle mass when compared with placebo (86785). However, a small crossover study in physically active females undergoing an anaerobic exercise test shows that taking a single dose of oral yohimbine 2.5 mg 20 minutes prior to repeated sprints increases average power output, total work, and heart rate and decreases average fatigue index when compared with placebo. Additionally, taking yohimbine decreases lactic acid levels and increases epinephrine levels when compared with baseline or placebo (107849). Due to the single-dose nature of this study, the effects of continued administration of yohimbine on athletic performance are unclear. The effects of yohimbe bark have not been investigated in clinical research.
• Depression. It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial in patients with depression. Details: A very small clinical study in patients with treatment-resistant major depressive disorder shows that taking yohimbine, a constituent of yohimbe, 30 mg daily for 10 days along with desipramine does not improve symptoms of depression when compared with desipramine alone (86834). Also, a small crossover trial in patients with major depressive disorder and/or a history of adverse childhood experiences shows that taking yohimbine 10 mg once does not alter approach-avoidance behaviors normally associated with depression when compared with placebo (111402). The effects of yohimbe bark have not been investigated in clinical research.
• Dry mouth. It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial for dry mouth. Details: A very small clinical study shows that taking yohimbine, a constituent of yohimbe bark, 18 mg daily for 5 days improves symptoms of dry mouth in individuals treated with antidepressants when compared with placebo (86886). The effect of yohimbe bark has not been investigated in clinical research.
• Erectile dysfunction (ED). It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial for ED. Details: The effects of yohimbe bark have not been investigated in clinical research. Most small clinical studies suggest that taking yohimbine, a constituent in yohimbe bark, seems to improve symptoms in adults with various types of ED, including organic, psychogenic, mixed, and others (3305,3307,3309,3311,3312,3313,10629,86827,86831,86878)(86882,86888,86896). A meta-analysis in patients with ED shows that taking 5-100 mg of oral yohimbine for 2-10 weeks increases the probability of erectile function improvement when compared with placebo. Also, the use of yohimbine in combination with either L-arginine or trazodone, but not as monotherapy, improves sexual function when compared with placebo (107848). These findings are limited by the high heterogeneity of the included studies. Due to insufficient data, guidelines from the American Urological Association do not recommend yohimbine as a treatment option for ED (86881).
• Fatigue. Although there is interest in using oral yohimbe for fatigue, there is insufficient reliable information about the clinical effects of yohimbe for this purpose.
• Obesity. Although there is interest in using oral yohimbe for obesity, there is insufficient reliable information about the clinical effects of yohimbe for this condition.
• Orthostatic hypotension. It is unclear if oral yohimbine, a constituent of yohimbe, is beneficial in patients with orthostatic hypotension. Details: Some preliminary clinical research shows that taking a single dose of yohimbine 5.4 mg increases systolic blood pressure and standing diastolic blood pressure in patients with orthostatic hypotension when compared with placebo or pyridostigmine (86811,86890). However, other preliminary clinical research shows that taking yohimbine does not improve blood pressure in patients with orthostatic hypotension due to autonomic failure when compared with placebo (86821). The effect of yohimbe bark has not been investigated in clinical research. More evidence is needed to rate yohimbe for these uses.

POSSIBLY UNSAFE ...when used orally. Yohimbine, a constituent of yohimbe, has been associated with serious adverse effects including cardiac arrhythmia, agitation, myocardial infarction, seizure, and others (17465). Some research shows that yohimbine can be safely used under close medical supervision for up to 10 weeks (3305,3307,3311,3313). However, due to safety concerns, yohimbe should not be used without medical supervision.

However, this interaction has not been reported in humans.

IMMUNOSUPPRESSANTS Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = B Theoretically, concurrent use of cordyceps might interfere with immunosuppressive therapy.  Details Animal and in vitro research suggests that cordyceps stimulates the immune system (3403,3404,3414,3431,3432). However, limited clinical research suggests that taking cordyceps may lower the necessary therapeutic dose of the immunosuppressant cyclosporine (92828), which suggests that cordyceps may have an immunosuppressive effect.

TESTOSTERONE Interaction Rating = Minor Be watchful with this combination. Severity = Mild •  Occurrence = Possible •  Level of Evidence = D Theoretically, concurrent use of cordyceps and testosterone might have additive effects.  Details Animal research suggests that cordyceps can increase testosterone levels (46087). ANTICOAGULANT/ANTIPLATELET DRUGS Interaction Rating = Minor Be watchful with this combination. Severity = Mild •  Occurrence = Possible •  Level of Evidence = D Theoretically, combining yohimbe bark with antiplatelet or anticoagulant drugs might have additive effects; however, this has not been reported in clinical research.  Details Research in healthy adults shows that taking yohimbine, a constituent of yohimbe bark, in doses of 8 mg or more, seems to inhibit platelet aggregation in vitro by binding to the alpha-2 adrenoceptor (86773,86806,86835,86853). The effects of yohimbe bark itself are unclear; yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed (86862,89263).

ANTIHYPERTENSIVE DRUGS Interaction Rating = Moderate Be cautious with this combination. Severity = Moderate •  Occurrence = Probable •  Level of Evidence = D Theoretically, yohimbe might reduce the effects of antihypertensive drugs.  Details Yohimbine, a constituent of yohimbe, is an alpha-2 adrenoceptor antagonist and has been reported to increase blood pressure in clinical research. Theoretically, concomitant use of yohimbe and antihypertensive drugs can interfere with blood pressure control (11,17465).

CLONIDINE (Catapres) Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = D Theoretically, yohimbe might precipitate clonidine withdrawal.  Details Chronic clonidine use can downregulate alpha-2 adrenoreceptors. Animal research and one human case report suggest that concomitant administration of yohimbine, an alpha-2 adrenoceptor antagonist, may precipitate clonidine withdrawal and lead to sympathomimetic toxicity, including hypertensive crisis (111406).

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES Interaction Rating = Minor Be watchful with this combination. Severity = Moderate •  Occurrence = Possible •  Level of Evidence = D Theoretically, CYP3A4 inhibitors might increase the levels and adverse effects of yohimbine, a constituent of yohimbe bark.  Details In vitro and clinical research shows that the yohimbe bark constituent, yohimbine, is metabolized by CYP3A4 enzymes (105688,105698). Theoretically, drugs that inhibit CYP3A4 might increase the levels and adverse effects of yohimbine.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES Interaction Rating = Minor Be watchful with this combination. Severity = Mild •  Occurrence = Possible •  Level of Evidence = D Theoretically, yohimbe might decrease the levels and clinical effects of CYP3A4 substrates.  Details In vitro research shows that yohimbe extract induces CYP3A4 enzymes (111404).

MONOAMINE OXIDASE INHIBITORS (MAOIs) Interaction Rating = Major Do not take this combination. Severity = High •  Occurrence = Likely •  Level of Evidence = D Concomitant use of MAOIs with yohimbe can result in additive effects.  Details Yohimbine, a constituent of yohimbe, has MAO inhibitory effects. At high doses, yohimbine is a non-selective inhibitor of MAO (11,12).

PHENOTHIAZINES Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = D Theoretically, using yohimbine with phenothiazines might have additive effects.  Details Yohimbine, a constituent of yohimbe, has alpha-2 adrenergic antagonist effects. Theoretically, combining it with phenothiazines can cause additive alpha-2 adrenergic antagonism (19).

STIMULANT DRUGS Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = D Theoretically, taking yohimbe with stimulant drugs can have additive effects.  Details Yohimbine, a constituent of yohimbe, has sympathomimetic effects and increases blood pressure in a dose-dependent manner. Theoretically, taking yohimbe with stimulant drugs can have additive stimulant and hypertensive effects (11,12,105698).

TRICYCLIC ANTIDEPRESSANTS (TCAs) Interaction Rating = Moderate Be cautious with this combination. Severity = High •  Occurrence = Possible •  Level of Evidence = D Theoretically, taking yohimbe with TCAs can increase adverse effects.  Details A small clinical study in patients taking TCAs for at least 4 weeks shows that receiving doses of intravenous yohimbine 2.5-20 mg daily for up to 7 days precipitates severe anxiety, agitation, and tremor (105881). The effects of yohimbe bark itself are unclear; oral yohimbe bark contains 0.6% to 1.38% yohimbine, but it is unclear how much is absorbed (86862,89263).

General ...Orally, cordyceps seems to be generally well tolerated when used for up to 1 year.
Most Common Adverse Effects:
Orally: Abdominal discomfort, constipation, diarrhea.

Gastrointestinal ...Orally, cordyceps has been associated with diarrhea, constipation, abdominal discomfort, dry mouth, and throat discomfort in clinical research. However, these events were uncommon, and in some cases symptoms could be reduced by taking cordyceps after eating (92829,105076,109705).

Hematologic ...Two cases of lead poisoning, characterized by loss of appetite and other symptoms, have been reported for patients taking cordyceps powder. After discontinuing cordyceps supplementation, both patients were treated with chelating agents (46135).

Hepatic ...There is a case report of acute cholestatic hepatitis probably associated with the use of a product containing cordyceps. The 64-year-old male was asymptomatic except for jaundice and laboratory markers and recovered once the supplement was stopped. However, it is unclear whether the hepatitis is associated with the cordyceps or with an unknown contaminant (109704).

Renal ...One case of a mild increase in serum creatinine level (< 30%) has been reported (95905).

General ...Orally and topically, pyruvate seems to be well tolerated.
Most Common Adverse Effects:
Orally: Bloating, diarrhea, and flatulence.
Topically: Burning sensation at high concentrations.

Cardiovascular ...Orally, pyruvate may increase triglyceride levels. In a clinical trial, pyruvate 5 grams twice daily for 30 days before exercise caused a 28% increase in plasma triglycerides when compared with placebo. It is unknown if the increase was temporary or sustained (13660).
Intravenously, pyruvate has been associated with one death following a pyruvate-loading test in a child with restrictive cardiomyopathy (2473).

Dermatologic ...Topically, application of a facial peel containing pyruvic acid 50% can cause an intense burning sensation on the skin and should only be applied over small areas (12020).

Gastrointestinal ...Orally, pyruvate may cause bloating, diarrhea, flatulence, and loose stools. In a clinical trial, pyruvate 22-44 grams daily was associated with gas and bloating in 19% more subjects than placebo and loose stools or diarrhea in 12% and 35% more subjects than placebo, respectively (2474). In another clinical trial, diarrhea, flatulence, and loose stool occurred in 58% of subjects taking pyruvate and 28% of subjects taking placebo (70259).

Pulmonary/Respiratory ...Topically, application of a facial peel containing pyruvic acid 50% can release vapors that are irritating to the respiratory tract. The application should be conducted in a room with adequate ventilation (12020).

General ...Orally, there is limited information available about the adverse effects of yohimbe. Yohimbine, a constituent of yohimbe, might be unsafe; most reported adverse effects are dose-related.
Most Common Adverse Effects:
Orally: Yohimbine, a constituent of yohimbe, has been associated with anxiety, agitation, diaphoresis, diarrhea, flushing, headache, hypertension, increased urination, nausea, tachycardia, tremors, vertigo, and vomiting.
Serious Adverse Effects (Rare):
Orally: Yohimbine, a constituent of yohimbe, has been associated with atrial fibrillation, hypertensive crisis, myocardial infarction, and QT interval prolongation.

Cardiovascular ...Orally, yohimbine, a constituent of yohimbe, has been associated with hypertension, especially at higher doses (3312,17465,86801,86802,86804,86811,86820,86822,86834,86856)(86786,86896). A case of hypertensive crisis was reported in a 63-year-old male taking a yohimbine-containing herbal product once daily for one month. The patient was successfully managed with intravenous nitroprusside followed by clonidine (91521). Tachycardia, fluid retention, palpitations, and chest discomfort have also been reported (3312,17465,86786,86793,86801,86802,86804,86822,86843,86854)(86856,86866,86867,86869,86871,86874,86875). Conduction abnormalities have also been reported (86856,86786). There have been some reports of myocardial infarction, atrial fibrillation, and QT interval prolongation (17465). In theory, these effects may also occur with the use of yohimbe bark extract.

Dermatologic ...Orally, yohimbine, a constituent of yohimbe, may cause rash, erythrodermic skin eruption, and exanthema (3312,3971,86804,86896,86878).

Gastrointestinal ...Orally, yohimbine, a constituent of yohimbe, may cause nausea, vomiting, increased salivation, diarrhea, and gastrointestinal distress (3970,17465,49902,86780,86781,86786,86801,86804,86824,86827)(86828,86829,86863,86878,86882,86896).

Genitourinary ...Orally, yohimbine may cause dartos contraction or decreased libido in some patients (86786,86882). A case of severe intractable priapism has been reported for a 42-year-old male who took a supplement containing yohimbe extract the previous day for sexual enhancement. Treatment with phenylephrine 400 mcg was unsuccessful at resolving the priapism, so surgical insertion of a proximal cavernosal spongiosum shunt was needed (86804).

Hematologic ...A case of drug-induced agranulocytosis has been reported following prolonged use of oral yohimbine, a constituent of yohimbe (86877).

Immunologic ...There is one report of a hypersensitivity reaction including fever; chills; malaise; itchy, scaly skin; progressive renal failure; and lupus-like syndrome associated with ingestion of a one-day dose of yohimbine, a constituent of yohimbe (6169).

Musculoskeletal ...Orally, yohimbine, a constituent of yohimbe, may cause muscle aches (86850).

Neurologic/CNS ...Orally, yohimbine, a constituent of yohimbe, has been associated with reports of general central nervous system (CNS) and autonomic excitation, tremulousness, head twitching, seizure threshold changes, enhanced brain norepinephrine release, decreased energy, dizziness, vertigo, and headache (3312,3971,86774,86779,86786,86804,86827,86857,86870,86882)(86883). Cold feet and chills have also been reported with yohimbine (86827,86896). Other adverse reactions include flushing and diaphoresis (17465). Excessive doses of yohimbine can also cause paralysis (11,18). A case of acute neurotoxicity characterized by malaise, vomiting, loss of consciousness, and seizures has been reported for a 37-year-old bodybuilder who ingested a single dose of yohimbine 5 grams. Improvement was seen within 12 hours following treatment with furosemide, labetalol, clonidine, urapidil, and gastrointestinal decontamination (86801).

Psychiatric ...Orally, yohimbine, a constituent of yohimbe, may increase malaise, fatigue, insomnia, restlessness, agitation, and anxiety (3312,3970,3971,17465,86786,86801,86804,86822,86827,86834)(86868,86878,86882,86896). In a clinical study of healthy subjects, administration of yohimbine increased impulsivity, with larger doses increasing impulsivity more than 50% (86784,86810).

Pulmonary/Respiratory ...Orally, yohimbine, a constituent of yohimbe, may cause bronchospasm, tachypnea, cough, and rhinorrhea (17465,86825,86850). A case of sinusitis characterized by pain and discomfort above both eyes has been reported for a 59-year-old male taking yohimbine 5.4 mg three times daily to treat erectile dysfunction. Symptoms resolved within 24 hours of discontinuing yohimbine. The effect was attributed to the alpha-2 adrenergic antagonist effects of yohimbine (94112). Excessive doses of yohimbine can cause respiratory depression (1118).

Renal ...Orally, yohimbine, a constituent of yohimbe, may increase urinary frequency (3312,3970,3971,17465,86804,86827,86850,86861,86882). A case of acute renal failure has been reported for a 42-year-old male taking yohimbine. Normalization of renal function was achieved following 2 weeks of treatment with corticosteroids. The renal dysfunction was attributed to yohimbine-induced systemic lupus erythematosus (6169).
